An 82-year-old man with Alzheimer disease begins wandering at night and resisting redirection. His daughter, exhausted after two weeks of interrupted sleep, asks for more lorazepam or an antipsychotic. During examination, he grimaces when his right knee is moved. Treating his agitation without investigating that grimace could miss the most actionable finding. [\[1\]](#cite-1 "Reference [1]")

The first decision: distress, delirium, or both?
------------------------------------------------

### Read the timeline before the laboratory results

A two-week behavioral change warrants assessment for delirium superimposed on dementia, even with stable vital signs, a normal CBC and metabolic panel, and an unremarkable urinalysis. Ask his daughter about baseline attention, fluctuations, medication changes, falls, bowel habits, sleep, and new functional losses. Examine for altered alertness and inattention; if delirium indicators emerge, use a validated assessment such as the 4AT and investigate the cause. [\[2\]](#cite-2 "Reference [2]")

The differential remains practical rather than exhaustive:

- **Pain or discomfort:** Knee pathology is plausible; also assess for constipation, urinary retention, dental pain, and pressure injury.
- **Delirium:** Look for an acute change in attention or awareness, particularly a fluctuating course.
- **Medication effects:** Review lorazepam use, recent additions, anticholinergic burden, and adverse effects.
- **Environmental distress:** Identify what happens immediately before wandering or aggression, including noise, fatigue, or difficult redirection.

A negative urinalysis does not close the diagnostic workup, and a positive one would not, by itself, establish the cause of agitation. In this patient, reproducible knee discomfort makes pain assessment and treatment the most appropriate *first intervention*, alongside evaluation for delirium. NICE recommends structured observational pain assessment when self-report is unreliable and reassessment after treatment. [\[1\]](#cite-1 "Reference [1]")

Treat what the behavior may be communicating
--------------------------------------------

### Test the pain hypothesis

Assess the knee clinically, clarify whether movement or walking triggers distress, and investigate further if the examination suggests injury or another condition requiring treatment. An appropriately selected trial of regular paracetamol (acetaminophen) may help if musculoskeletal pain is suspected; account for liver disease, combination products, and local prescribing guidance. Record pain-related behavior and agitation before and after treatment rather than assuming improvement or failure from one difficult evening. [\[1\]](#cite-1 "Reference [1]")

Alzheimer pathology can make it harder for a patient to explain discomfort or understand why someone is moving a painful joint. Resistance may therefore be an intelligible response to pain, not evidence that sedation is required. Donepezil serves a different purpose: reversible acetylcholinesterase inhibition increases available acetylcholine, partly addressing cholinergic dysfunction; it does not reverse neurodegeneration. [\[1\]](#cite-1 "Reference [1]")

> **Clinical pearl:** When aggression occurs during hands-on care, observe the task and examine the body part involved before escalating psychotropics. [\[1\]](#cite-1 "Reference [1]")

### Build a behavioral formulation with the caregiver

Ask the daughter to keep a brief antecedent–behavior–consequence record: What preceded the outburst, what did he do, and how did others respond? Use it to distinguish pain during transfers from distress caused by confrontation at the door. Then agree on one or two changes—such as calmer redirection, a predictable routine, or an activity matched to his interests—and review whether they work. NICE recommends psychosocial and environmental interventions as initial *and ongoing* care. [\[1\]](#cite-1 "Reference [1]")

When medication becomes a safety decision
-----------------------------------------

### Explain why more lorazepam is not the answer

Increasing lorazepam to suppress nighttime wandering may leave him less steady without addressing why he gets up. Discuss three material risks: worsened attention and memory or disinhibition; sedation and ataxia leading to falls and fractures; and respiratory suppression, particularly with other sedatives or respiratory vulnerability. Review how often he actually takes the PRN dose and plan any reduction with attention to dependence and withdrawal rather than stopping regular use abruptly. [\[3\]](#cite-3 "Reference [3]")

### Reserve an antipsychotic for serious harm or distress

Suppose a trial of pain treatment and individualized behavioral measures does not help, and he begins striking his daughter. First secure immediate safety and reassess for new medical triggers. If aggression persists and there is a risk of harm, **risperidone** is licensed in the UK for up to six weeks for persistent aggression in moderate to severe Alzheimer dementia when non-drug approaches have failed; that indication does not make it a treatment for wandering alone. [\[1\]](#cite-1 "Reference [1]")

Discuss the intended behavioral target, limited duration, and alternatives with the patient as far as possible and with his daughter as appropriate. Counsel explicitly about increased **mortality** and **stroke or other cerebrovascular events**, alongside sedation, falls, and movement-related adverse effects. If prescribed, use the lowest effective dose for the shortest possible time, monitor benefit and harm, and reassess at least every six weeks; stop if there is no clear ongoing benefit. Antipsychotic prescribing and licensing vary by country. [\[1\]](#cite-1 "Reference [1]")

The caregiver is part of the care plan
--------------------------------------

### Address the next night—and the months ahead

His daughter’s exhaustion is an urgent clinical concern, not evidence that she has failed. Offer tailored psychoeducation and skills training in responding to behavior, and arrange assessment for respite and other practical support. Ask directly whether she can keep both of them safe tonight; if not, organize urgent local assistance rather than relying on a prescription to make home care viable. [\[1\]](#cite-1 "Reference [1]")

For late-afternoon agitation with sleep–wake reversal, establish consistent daytime activity and exposure to daylight, alongside exercise, sleep hygiene, and a calmer evening routine suited to his abilities. Check again for pain or other unmet needs when symptoms peak: “sundowning” describes a pattern, not its cause. NICE recommends a personalized, multicomponent sleep approach rather than melatonin for insomnia in Alzheimer disease. [\[1\]](#cite-1 "Reference [1]")

Key Points for Board Exams
--------------------------

- A sudden change in dementia calls for delirium assessment and a search for treatable triggers; normal initial tests do not replace bedside reasoning. [\[2\]](#cite-2 "Reference [2]")
- Grimacing with knee movement makes structured pain assessment and treatment the first move in this vignette. [\[1\]](#cite-1 "Reference [1]")
- Donepezil inhibits acetylcholinesterase; lorazepam can worsen cognition and increase fall risk. [\[4\]](#cite-4 "Reference [4]")
- UK-licensed short-term risperidone applies to *persistent, harmful aggression*, not routine nighttime wandering. Discuss mortality and cerebrovascular risk. [\[1\]](#cite-1 "Reference [1]")
- Behavioral observation, tailored activity, caregiver training, respite planning, and regular review remain central even if medication is used. [\[1\]](#cite-1 "Reference [1]")

The clinical goal is not simply a quieter night. It is to find what changed, reduce preventable harm, and create a plan the patient and his daughter can sustain. [\[1\]](#cite-1 "Reference [1]")

Frequently Asked Questions
----------------------------

### Do normal blood tests and a negative urinalysis rule out delirium here?

No. Assess attention, alertness, fluctuation, medications, pain, and other potential causes; use a validated delirium assessment when indicated. [\[2\]](#cite-2 "Reference [2]")

### Why prioritize the right knee when the presenting complaint is aggression?

Grimacing and resistance during movement suggest pain he may struggle to report. Assess and treat the suspected cause, then reassess both pain and behavior. [\[1\]](#cite-1 "Reference [1]")

### Is risperidone licensed in the UK for nighttime wandering?

No. Its relevant UK indication is short-term treatment, up to six weeks, of persistent aggression in moderate to severe Alzheimer dementia when non-drug approaches have failed and harm is a risk. [\[5\]](#cite-5 "Reference [5]")

### What support should the daughter receive beyond advice on redirection?

Offer tailored psychoeducation and behavioral skills training, assess her own health and safety needs, and arrange assessment for respite and other practical support. [\[1\]](#cite-1 "Reference [1]")
