  It is easy to spend weeks reading psychiatry and still feel unprepared for the boards. The problem is often not effort; it is practicing one topic at a time when the American Board of Psychiatry and Neurology (ABPN) asks you to make decisions across diagnoses, mechanisms, and treatments. Start by matching your study plan to the **initial certification** exam—not a continuing-certification course or a generic psychiatry review. [\[1\]](#cite-1 "Reference [1]")

Build Your Plan Around the Initial Certification Blueprint
----------------------------------------------------------

The ABPN initial psychiatry certification exam has **425 questions in eight sections**. Its blueprint has two overlapping dimensions: psychiatric disorders and topics, and the competencies or mechanisms assessed within them. A substance-use question, for example, might test diagnosis, treatment, or systems-based practice. Use both dimensions when reviewing each practice question. [\[1\]](#cite-1 "Reference [1]")

### Prioritize without creating blind spots

The disorder blueprint assigns substantial ranges to psychotic disorders and depressive disorders (8–12% each), substance-related disorders (7–11%), anxiety disorders (7–9%), and neurocognitive disorders (6–8%). Treatment accounts for 25–35% of the competency dimension; neuroscience and mechanisms account for 17–23%. These are **ranges, not a prediction** of your particular form. - Start each week with one higher-weight disorder group. For every diagnosis, practice the distinguishing features, relevant medical or substance-related alternatives, and treatment decisions.
- Reserve a separate block for neurology, neurodevelopment, sleep, ethics, and forensic psychiatry. Mark them on your blueprint checklist so familiar outpatient topics do not crowd them out.
- When an explanation exposes a gap, label it twice—for example, “neurocognitive disorders / diagnostic procedures.” This shows whether you need a topic review, a reasoning review, or both.

Turn Questions into Clinical Decisions
--------------------------------------

Use an exam-style question bank as your main practice resource, not as a race to finish questions. After each set, write down the clue that determined the answer and why your strongest alternative was wrong. For a patient with new agitation, for example, make yourself identify the evidence for a primary psychiatric disorder versus a medical or substance-related cause before choosing management.

1. Do a timed set of 20–30 mixed questions twice weekly. Record errors as knowledge gaps, misread clues, or premature answers.
2. Review incorrect answers and correct guesses on the same day. Use a concise textbook or authoritative clinical reference only for the specific gap you found.
3. Revisit your error log later in the week. Answer the original question again *before* reading your note; then explain the decision aloud in one sentence.

> **Pro Tip:** A rising question-bank percentage can reflect familiarity with repeated items. Test progress with previously unseen, mixed questions and compare your error categories—not just your score.

The ABPN does not prescribe a commercial question bank or a passing percentage for your practice sets. Treat practice scores as feedback for choosing the next topic, not as a guarantee about the examination. The actual exam uses a total standard score, with no predefined passing rate for a group. Rehearse Linked Cases and the Exam Clock
----------------------------------------

Initial certification alternates stand-alone sections with linked-item-set sections. Linked sets may use text, audio, or video and can require more than one answer. In a linked set, you must answer before moving forward; once you advance, you can look back but **cannot change that answer**. Stand-alone items can be flagged for review within their section, but you cannot return to either type after leaving the section. [\[2\]](#cite-2 "Reference [2]")

Practice linked cases as a sequence: identify the presentation, commit to each answer, then interpret any new information without rewriting your earlier decision. For multi-answer questions, check the requested number of selections before advancing. Ask a study partner to present a brief case aloud occasionally, so you practice retaining key findings without rereading a transcript.

The exam allows **500 minutes for questions** and up to **60 minutes of pooled breaks**, separate from that question time. As a starting checkpoint, 500 minutes across 425 questions is about 71 seconds per question on average; media cases will need more, so do not force that pace on every item. As of October 2026, the ABPN lists **September 7–8, 2027** for the next psychiatry specialty certification examination. Confirm your own eligibility, application deadlines, appointment, and current exam documents with ABPN before fixing your final 12-week calendar. - **Reading without retrieval:** After a chapter, answer related cases and explain why a plausible distractor fails.
- **Studying only your clinical niche:** Compare your completed topics with every category in the initial-certification blueprint.
- **Treating linked items like ordinary flagged questions:** Practice committing before advancing; do not rely on changing an earlier answer.
