  Knowing an OMT technique is not the same as demonstrating it clearly under a timer. Candidates often rehearse the treatment but forget to name landmarks, explain their reasoning, or adapt when their first diagnosis is wrong. For this exam, practice the entire performance—not just the hand placement.

Learn the exam sequence before rehearsing techniques
----------------------------------------------------

The AOBFP OMT Performance Exam uses three cases: one involving the spine, one the extremities, and one systemic disease. You have **15 minutes to review all three case sheets**, select a diagnosis from four choices for each, and plan treatment. At the stations, you have **four minutes per case** to demonstrate and discuss OMT. You must pass two of the three cases. [\[1\]](#cite-1 "Reference [1]")

Make your first practice session a faithful simulation. Print three short cases, prohibit reference materials, set a 15-minute preparation timer, then run three four-minute demonstrations with a colleague acting as the patient. The official AOBFP retired sample case is a useful model for the amount of clinical information provided; do not mistake it for a prediction of future cases. [\[1\]](#cite-1 "Reference [1]")

During the 15 minutes, use a repeatable workflow:

1. Read all three cases and mark findings that distinguish the four diagnoses.
2. Choose each answer before refining any treatment plan.
3. For each case, note an appropriate technique, relevant landmarks, patient position, and one sentence explaining why it fits.
4. Use remaining time to check safety and rehearse your opening words mentally.

Build a demonstration that makes your skills visible
----------------------------------------------------

AOBFP scoring covers diagnosis, landmark identification, technique demonstration, discussion, and professionalism. Examiners cannot credit an explanation they cannot hear or a landmark you never identify. Practice speaking while you work, rather than silently performing a polished maneuver. ### Use the same sequence at every station

- **Orient:** State the diagnosis and name the technique you intend to demonstrate.
- **Locate:** Name and palpate the relevant landmarks; explain what their positions mean for this case.
- **Position:** Place the patient and your hands deliberately, describing the setup.
- **Explain:** State the direction or timing of the technique and why it is appropriate.
- **Close:** Check the patient's comfort and finish your explanation before time is called.

Ask your practice partner to score only what they observed or heard. After each run, have them identify one omitted landmark, one unclear explanation, and one adjustment to patient positioning. Record yourself occasionally: a short video can reveal whether your hands obscure the landmark you are naming or your voice fades while you position the patient.

Prioritize breadth, then repair your weakest technique
------------------------------------------------------

Passing two cases does not make the third category optional. Rotate through spine, extremity, and systemic-disease cases each week so an unfamiliar presentation does not consume your preparation time. Within each category, choose techniques you can demonstrate accurately and explain clearly, rather than assembling an impressive but unreliable catalog. [\[2\]](#cite-2 "Reference [2]")

Use resources according to the skill they can actually teach:

- **Official AOBFP exam-day guidance and sample case:** Learn the instructions, case layout, timing, and scoring language; turn each criterion into a practice checklist. [\[1\]](#cite-1 "Reference [1]")
- **Osteopathic technique texts:** Verify anatomy, positioning, and technique mechanics after a rehearsal exposes a gap. Do not substitute reading for hands-on practice.
- **Supervised hands-on teaching:** Ask an experienced osteopathic clinician to correct palpation and patient positioning while you explain your choices aloud.
- **Case questions or a question bank:** Use short sets to practice distinguishing diagnoses, then immediately demonstrate an appropriate technique for the answer you selected.

Include clinical judgment in systemic cases. For example, the retired sample describes postoperative abdominal distention; first identify the most likely diagnosis from the supplied findings, then discuss an appropriate OMT approach. In real practice, a deteriorating postoperative patient needs clinical evaluation, not an exam-style treatment script. [\[3\]](#cite-3 "Reference [3]")

If you are starting several weeks before the exam, use this four-week cycle. Repeat the middle weeks if you have longer; shorten reading time, not supervised practice, if you have less.

After every session, keep an error log with five columns: diagnosis, landmarks, setup, explanation, and professionalism. Mark the first point where your performance broke down, then rehearse that step before attempting another full case. This is more efficient than repeatedly running a technique you already know.

> **Pro Tip:** During the exam, an active correction, thrust, or HVLA maneuver must **not** be completed. Set up the patient and verbalize the mechanism and direction instead; performing the active correction is grounds for immediate failure of the entire exam. Common pitfalls to remove now
-----------------------------

- **Preparing like the written exam:** Multiple-choice review helps diagnosis, but cannot show whether you palpate, position, and explain effectively.
- **Overinvesting in one difficult case:** Commit to all three diagnoses during the 15-minute review; each case offers a chance to demonstrate your skills.
- **Being inaudible or rushing patient interaction:** Have your partner stand where an examiner might sit and confirm that every landmark and rationale is understandable.
- **Ignoring exam-day logistics:** Confirm your assigned time and partner arrangements. AOBFP requires a qualifying partner or assigns a surrogate when needed; review its identification, attire, and room rules in advance. [\[1\]](#cite-1 "Reference [1]")
