  A shocked patient has a radial arterial line reading 210/40 mm Hg while the cuff reads 88/54. Before changing the vasopressor, inspect the system. An arterial line is not simply a catheter—it is an access procedure, a fluid-filled measuring circuit, and a waveform requiring interpretation.

Current as of September 2026, these technique basics emphasize conceptual awareness rather than independent procedural credentialing.

Think in Three Linked Systems
-----------------------------

Approach every arterial line through three questions:

1. **Was the artery entered safely?** Consider anatomy, sterility, and needle-tip control.
2. **Was the catheter connected securely?** Prevent dislodgement, hemorrhage, air, and clot.
3. **Is the displayed pressure trustworthy?** Level, zero, and assess the waveform before treating the number.

Failure in any layer can produce infection, distal ischemia, blood loss, or inappropriate resuscitation. [\[1\]](#cite-1 "Reference [1]")

Aseptic Technique Is Part of the Procedure
------------------------------------------

Do not downgrade sterility because the artery is peripheral. Perform hand hygiene, prepare equipment before puncture, and use sterile gloves. For peripheral arterial catheter insertion, CDC guidance specifies at least a cap, mask, sterile gloves, and a small sterile fenestrated drape; femoral or axillary placement requires maximal sterile barrier precautions.

Prepare clean skin with greater than 0.5% chlorhexidine in alcohol and let it dry completely. Use an approved alternative when chlorhexidine is contraindicated. Once prepared, do not repalpate the site unless sterility is maintained. [\[2\]](#cite-2 "Reference [2]")

After cannulation:

- Cover the site with a sterile transparent dressing, or gauze if bleeding persists.
- Keep the insertion site visible whenever possible.
- Remove the catheter when continuous monitoring or repeated sampling is no longer necessary.
- If emergency conditions compromised asepsis, document this and arrange prompt replacement according to institutional policy.

Ultrasound: See the Tip, Not Just the Artery
--------------------------------------------

Ultrasound is particularly valuable during hypotension, obesity, edema, weak pulses, prior failed attempts, and pediatric access. Evidence suggests real-time B-mode guidance improves first-attempt and overall radial artery cannulation success compared with palpation alone. [\[4\]](#cite-4 "Reference [4]")

Begin with a preprocedural scan. Identify the artery and adjacent veins, assess depth and caliber, and look for thrombosis, calcification, or an overlying hematoma. Confirm pulsatility and compressibility rather than relying on color Doppler alone.

Both common approaches have trade-offs:

Whichever approach you use, keep the needle tip under direct control. Seeing tissue movement or a flash of blood does not prove that the catheter is safely intraluminal. Never advance against resistance; reassess the image, angle, and catheter position.

Securement Protects the Patient and the Signal
----------------------------------------------

Confirm pulsatile blood return and an arterial waveform before final securement. Use a sterile dressing and a compatible sutureless securement device when available. Add a gentle loop of tubing for strain relief, but avoid excess tubing that can snag during transport.

The connection must remain closed, clearly labeled, and free of air. Secure the wrist in a neutral position when movement repeatedly kinks a radial catheter, but reassess distal color, temperature, capillary refill, and pulse. Treat new pain, pallor, paresthesia, swelling, bleeding, or waveform loss as a complication—not merely an equipment nuisance. [\[2\]](#cite-2 "Reference [2]")

Make the Waveform Earn Your Trust
---------------------------------

Level the transducer at the right atrium for routine systemic arterial pressure measurement, then zero it to atmospheric pressure. Re-level after repositioning or stretcher transfer. A transducer below the reference point reads falsely high; one above it reads falsely low.

A normal trace typically shows a rapid systolic upstroke, systolic peak, dicrotic notch, and diastolic runoff. When the number conflicts with the patient, perform a fast-flush test and inspect the entire circuit. [\[5\]](#cite-5 "Reference [5]")

MAP is generally less distorted than systolic or diastolic pressure, but no value should be accepted blindly. Compare the trace with cuff pressure, pulse examination, perfusion, and the clinical trajectory.

- Treating an isolated arterial systolic value without checking waveform quality.
- Forgetting to re-level after changing bed height.
- Advancing because blood flashed despite losing the ultrasound needle tip.
- Contaminating the field with an uncovered probe or nonsterile gel.
- Ignoring gradual waveform damping caused by clot, air, kinking, or catheter-wall contact.
- Assuming a secured dressing eliminates the risk of disconnection and hemorrhage.
