Your patient has had three steroid bursts this year. The chart says “severe asthma,” but the new infiltrate, recurrent nasal polyps, or noisy inspiration may be telling a different story. Before escalating inhalers or selecting a biologic, ask whether another process is driving the symptoms. The 2026 GINA strategy emphasizes confirming asthma and investigating comorbidities and alternative diagnoses in difficult-to-treat disease. [\[1\]](#cite-1 "Reference [1]")

First, decide what the wheeze represents
----------------------------------------

Asthma can coexist with all three conditions discussed here. Allergic bronchopulmonary aspergillosis (ABPA) complicates airway disease; aspirin-exacerbated respiratory disease (AERD) identifies a distinct asthma-associated syndrome; and inducible laryngeal obstruction—often called vocal cord dysfunction (VCD)—can mimic asthma or occur alongside it.

### Three clues that should change your next test

- **New mucus plugging or fleeting opacities:** Investigate ABPA rather than labeling every deterioration an asthma exacerbation.
- **Recurrent nasal polyps plus respiratory reactions to aspirin or NSAIDs:** Ask specifically about AERD.
- **Abrupt inspiratory noise or throat tightness:** Consider VCD/inducible laryngeal obstruction, especially when bronchodilators do little.

First verify variable expiratory airflow limitation where possible, and check inhaler technique and adherence. Then investigate the clue in front of you; “refractory asthma” is not itself an explanation. [\[1\]](#cite-1 "Reference [1]")

ABPA: When fungal sensitization becomes lung injury
---------------------------------------------------

ABPA is an allergic response to *Aspergillus fumigatus* in the airways, not invasive fungal infection. Suspect it in a patient with asthma and recurrent exacerbations accompanied by mucus plugs, fleeting infiltrates, or bronchiectasis. Missing it matters: ongoing inflammation and plugging can contribute to structural airway damage. [\[2\]](#cite-2 "Reference [2]")

### Use the current criteria, not IgE alone

Order *A. fumigatus*–specific IgE, total IgE, blood eosinophils, and *Aspergillus*–specific IgG; obtain thin-section chest CT when ABPA is suspected. Under the 2024 ISHAM criteria, a compatible presentation or predisposing condition generally requires *A. fumigatus*–specific IgE ≥0.35 kUA/L, total IgE ≥500 IU/mL, **and two** of the following: positive specific IgG, eosinophils ≥500/µL (including a historical count), or characteristic imaging. Exceptions exist, so interpret results with the full clinical picture. [\[2\]](#cite-2 "Reference [2]")

> **Clinical Pearl:** High-attenuation mucus on chest CT is particularly compelling: ISHAM considers it pathognomonic for ABPA. Conversely, a high total IgE or a positive fungal sensitization test alone does not establish the diagnosis. [\[2\]](#cite-2 "Reference [2]")

For symptomatic acute ABPA, specialist-guided oral prednisolone **or** itraconazole is recommended initial therapy. Do not reflexively prescribe both together for a first episode; combination therapy is generally reserved for recurrent exacerbations. Follow symptoms, imaging, and total IgE during treatment, while continuing appropriate asthma care. [\[3\]](#cite-3 "Reference [3]")

AERD: The medication history is the diagnostic clue
---------------------------------------------------

Picture an adult with asthma, chronic rhinosinusitis with nasal polyps, impaired smell, and congestion or bronchospasm after ibuprofen. That pattern suggests AERD, also called NSAID-exacerbated respiratory disease. Reactions result from COX-1 inhibition in a leukotriene-rich inflammatory state; this is not simply an IgE-mediated allergy to one brand of analgesic. [\[4\]](#cite-4 "Reference [4]")

### Ask about exposures—and avoid an unsafe shortcut

Clarify which drug caused the reaction, how quickly respiratory symptoms appeared, and whether the patient has since tolerated aspirin or another COX-1–inhibiting NSAID. In an uncertain history, an aspirin challenge can establish the diagnosis, but it belongs in a supervised specialist setting—not as an informal outpatient trial. [\[4\]](#cite-4 "Reference [4]")

- Avoid aspirin and other COX-1–inhibiting NSAIDs until the evaluation establishes a safe plan.
- Treat both airway and sinonasal disease; options include inhaled and intranasal corticosteroids, leukotriene modifiers, and selected biologics or sinus surgery.
- Consider specialist-directed aspirin desensitization followed by maintenance aspirin for suitable patients. Weigh potential benefit against bleeding and gastrointestinal risks; desensitization is not a one-time cure. [\[4\]](#cite-4 "Reference [4]")

VCD: Listen for the phase of the noise
--------------------------------------

A patient who describes sudden throat closure during exercise may be said to “wheeze,” yet the loudest sound occurs on inspiration. VCD is episodic laryngeal narrowing, not lower-airway bronchoconstriction. Symptoms may also be triggered by irritants and can coexist with genuine asthma, so neither diagnosis should be dismissed without assessment. [\[5\]](#cite-5 "Reference [5]")

### Test during symptoms when possible

Laryngoscopy showing inappropriate inspiratory vocal-fold narrowing is the conventional diagnostic test; use provocation when needed to reproduce the patient’s symptoms. An inspiratory flow-volume loop abnormality may raise suspicion, but normal spirometry between episodes does not rule VCD out. For exercise-triggered episodes, continuous laryngoscopy during exercise can be particularly useful. [\[6\]](#cite-6 "Reference [6]")

Speech pathology–led breathing and laryngeal control techniques are the mainstay of management, alongside attention to relevant triggers and coexisting asthma. Avoid escalating systemic corticosteroids solely because inspiratory symptoms persist; first establish whether there is objective evidence of a lower-airway exacerbation. In an acutely distressed patient, however, assess the airway and other dangerous causes of stridor before settling on VCD. [\[5\]](#cite-5 "Reference [5]")

Key Takeaways
-------------

- In difficult-to-treat asthma, confirm airflow variability and investigate clues before escalating therapy.
- Think **ABPA** with mucus plugs, changing opacities, or bronchiectasis; apply the full diagnostic criteria.
- Think **AERD** when asthma, nasal polyps, and COX-1 NSAID respiratory reactions occur together.
- Think **VCD** with episodic inspiratory noise and throat tightness; seek laryngoscopy during symptoms.

The board-exam distinction is also the bedside distinction: ABPA needs an immunologic and imaging workup, AERD needs a careful drug history, and VCD needs attention to the upper airway. Each requires more than another inhaler.

Frequently Asked Questions
----------------------------

### Does an elevated total IgE confirm ABPA in a patient with asthma?

No. Diagnosis requires evidence of *Aspergillus* sensitization and additional compatible immunologic or imaging findings; elevated total IgE alone is nonspecific.

### Can a patient with AERD safely try aspirin at home to confirm the diagnosis?

No. Aspirin challenge can provoke significant respiratory reactions and should be performed under specialist supervision.

### Does normal spirometry between attacks exclude vocal cord dysfunction?

No. Episodic laryngeal obstruction may leave interval spirometry normal; laryngoscopy during symptoms or provocation is more informative.

### Can vocal cord dysfunction and asthma occur in the same patient?

Yes. Assess both conditions rather than assuming that evidence of one excludes the other.
