An 8 kg weight gain is troubling; new thirst makes this patient’s missed metabolic monitoring an immediate clinical problem. He is 45, takes clozapine 400 mg and aripiprazole 15 mg daily, and now has fatigue, a BMI of 32 kg/m², and blood pressure of 145/92 mmHg. His clinic’s audit reveals a parallel safety gap: only 30% of patients prescribed second-generation antipsychotics have documented glucose and lipid testing in the preceding year.

What needs explaining today?
----------------------------

### Separate the likely diagnosis from competing causes

New type 2 diabetes is a leading concern, but neither weight gain nor clozapine exposure establishes it. Polydipsia warrants prompt glucose assessment; ask about polyuria, visual changes, vomiting, dehydration, and altered consciousness to determine whether same-day urgent evaluation is needed. [\[1\]](#cite-1 "Reference [1]")

The focused differential remains broader:

- **Metabolic disease:** diabetes, dyslipidemia, and possible medication-associated weight gain.
- **Medication effects:** clozapine-related sedation or a change in exposure following altered smoking, illness, or interacting medication.
- **Other contributors:** sleep apnea, hypothyroidism, depression, or reduced activity; investigate according to the history and examination.

Clozapine’s metabolic liability reflects several pathways rather than one receptor. Histamine H1 and serotonin 5-HT2C antagonism are strongly implicated in increased appetite and weight gain; muscarinic M3 effects may contribute to disturbed glucose regulation. Concurrent aripiprazole does **not** remove the need for metabolic surveillance. [\[2\]](#cite-2 "Reference [2]")

Investigate the patient, not just the prescription
--------------------------------------------------

### Prioritize actionable tests

- Obtain **laboratory HbA1c or venous fasting plasma glucose** promptly to assess diabetes. If he appears acutely unwell, assess glucose immediately rather than waiting for a fasting appointment. [\[1\]](#cite-1 "Reference [1]")
- Request a lipid profile, including triglycerides, to characterize cardiovascular risk and guide follow-up. Document weight, waist circumference, pulse, and repeat blood pressure. [\[3\]](#cite-3 "Reference [3]")
- Check electrolytes, creatinine, and eGFR to assess renal health and inform subsequent diabetes or hypertension treatment. Add other fatigue investigations, such as TSH, when clinically indicated.
- Verify the **current absolute neutrophil count (ANC)** and its monitoring schedule. Do not infer that a gap in *metabolic* testing means ANC monitoring has stopped. In the United States, clozapine REMS ended on June 13, 2025, but prescribing-information ANC monitoring remains recommended. [\[4\]](#cite-4 "Reference [4]")

Review smoking, adherence, recent infection, and adverse effects before deciding whether a clozapine trough level would clarify altered exposure or possible toxicity. Arrange an ECG in light of cardiovascular findings and medication review, without treating an annual ECG as a universal substitute for metabolic testing. NICE recommends ECG assessment in specified risk circumstances and ongoing glucose and lipid monitoring during antipsychotic treatment. [\[5\]](#cite-5 "Reference [5]")

> **Clinical pearl:** An electronic order is not a completed test. Check that the patient can reach phlebotomy, that results return to a named clinician, and that abnormalities trigger action.

The abnormal result changes the conversation
--------------------------------------------

### Treat the disease while protecting psychiatric stability

Three weeks later, fasting plasma glucose is 8.2 mmol/L and triglycerides are substantially elevated. The glucose exceeds the diabetes diagnostic threshold of 7.0 mmol/L; obtain confirmatory laboratory testing unless hyperglycemia is unequivocal. Assess symptoms and the triglyceride value promptly rather than leaving either result until the next routine psychiatry visit. [\[1\]](#cite-1 "Reference [1]")

Explain the results without implying that the patient must choose between physical and mental health. Arrange primary-care or diabetes-team follow-up for glycemic assessment, blood-pressure confirmation, renal and cardiovascular risk review, and lipid management. Severe hypertriglyceridemia may require more urgent management; its urgency cannot be judged from the phrase “significant hypertriglyceridemia” alone.

His distress about weight deserves a treatment discussion, not reassurance alone. Offer supported nutrition and activity interventions, review the continuing rationale for aripiprazole augmentation, and consider **metformin** if appropriate after renal assessment; evidence supports a modest benefit for clozapine-associated weight gain. With confirmed type 2 diabetes and obesity, a weight-beneficial glucose-lowering treatment may also be appropriate through shared care. Avoid reflexively stopping effective clozapine before weighing relapse risk against metabolic harm. [\[6\]](#cite-6 "Reference [6]")

Turn one missed test into a safer system
----------------------------------------

### Follow the result through the workflow

The team’s proposed aim is specific: increase the proportion of active clinic patients taking second-generation antipsychotics with **both** glucose/HbA1c and lipid results documented within the previous 12 months from 30% to 80% within six months. Define the eligible population and where outside results will be recorded before tracking the measure. The target is a local improvement goal, not a guideline threshold. [\[7\]](#cite-7 "Reference [7]")

Map each handoff—identification, ordering, appointment, blood draw, result receipt, review, and patient notification. If orders are reliably placed but patients miss a separate phlebotomy visit, another ordering reminder will not address the principal bottleneck.

- Ask patients what makes attendance difficult, including transport, anxiety, and confusion about fasting.
- Test same-day venous phlebotomy or closer-to-home collection with primary care, with explicit responsibility for reviewing results.
- Offer navigation or peer support when patients want help attending appointments.
- Explore point-of-care screening only with an agreed pathway for laboratory diagnostic confirmation when needed. [\[8\]](#cite-8 "Reference [8]")

Track **process** measures such as the percentage of eligible visits with a completed blood draw or documented appointment. Track the **system outcome** against the aim: the percentage with both results available within 12 months. Add a balancing measure, such as patient-reported burden or delays in communicating abnormal results; later clinical measures might include treatment of newly identified diabetes. [\[9\]](#cite-9 "Reference [9]")

In a small Plan–Do–Study–Act test, assistants record BMI and blood pressure for one clinic session. During **Study**, compare collection rates with baseline and examine missed cases; during **Act**, adapt the workflow before expanding it. Shared accountability means documenting who orders, follows up, and explains results—not assuming that either psychiatry or primary care has silently taken ownership. [\[10\]](#cite-10 "Reference [10]")

Key Points for Board Exams
--------------------------

- Polydipsia on clozapine calls for prompt diabetes assessment, not attribution to medication alone.
- Confirm a fasting glucose of 8.2 mmol/L when hyperglycemia is not unequivocal.
- Verify ANC monitoring separately from the metabolic-monitoring audit.
- Distinguish an order from a completed draw, and a monitoring outcome from a clinical outcome.
- In PDSA, compare observed data with predictions during **Study**.

The clinical and governance decisions meet at the same point: an abnormal result must reach a patient and a clinician empowered to act. A safer service measures that closed loop, not merely how often it requests blood tests.

    Frequently Asked Questions
----------------------------

 ### Does the end of clozapine REMS mean ANC testing can stop?

No. The US REMS ended in June 2025, but prescribers should continue ANC monitoring according to clozapine prescribing information. [\[4\]](#cite-4 "Reference [4]")

### Does a fasting glucose of 8.2 mmol/L establish diabetes?

It exceeds the diagnostic threshold, but confirmatory laboratory testing is needed unless hyperglycemia is unequivocal. [\[1\]](#cite-1 "Reference [1]")

### Would more electronic blood-test orders fix this clinic’s monitoring gap?

Not by themselves. If orders are already placed, test changes that improve completed draws, result review, and patient follow-up. [\[9\]](#cite-9 "Reference [9]")
