Eight hours of washing rituals leave a 38-year-old man dependent on his parents for daily functioning. His Y-BOCS is 34 despite five SSRI trials, clomipramine, antipsychotic augmentation, and more than 150 hours of therapist-guided exposure and response prevention (ERP). With major depression also present, the neuromodulation committee must decide whether deep brain stimulation (DBS) offers a defensible path forward.

Is This Treatment-Refractory OCD?
---------------------------------

### Establish the primary driver of disability

His longstanding contamination fears and ritualized washing support OCD as the primary target. Depression is clinically significant—his Beck Depression Inventory score is 32—but improvement after ECT without corresponding OCD improvement helps distinguish the two treatment targets. ECT response neither establishes nor excludes DBS candidacy for OCD.

Before calling OCD refractory, the team should revisit potential explanations for persistent symptoms:

- **Inadequate treatment delivery:** Verify medication duration, tolerability, adherence, and whether ERP involved sustained exposure with response prevention rather than reassurance or accommodation.
- **Diagnostic overlap:** Separate intrusive obsessions from depressive rumination, psychotic conviction, or anxiety-driven avoidance without compulsions.
- **Barriers to ERP:** Assess family accommodation, avoidance between sessions, substance use, and cognitive or developmental factors that may require adapted therapy.

A normal MRI helps surgical planning but does not diagnose OCD. Baseline assessment should document symptom dimensions, functioning, depressive severity, suicide risk, and realistic goals that can be reassessed after implantation.

### Why referral is reasonable—but approval is not automatic

He is an adult with 15 years of OCD, marked impairment, severe Y-BOCS symptoms, multiple adequate medication attempts, and unusually extensive ERP. These features closely match specialist selection frameworks; the International OCD Foundation describes criteria including at least five years of primary OCD, Y-BOCS of at least 28, adequate serotonin-reuptake inhibitor trials, antipsychotic augmentation, and ERP. Criteria vary by program, and neither his GAF of 35 nor ECT exposure is a universal requirement. [\[1\]](#cite-1 "Reference [1]")

His depression requires active assessment rather than reflexive exclusion. The committee must determine whether current suicidality, impaired judgment, another psychiatric condition, or medical risk would undermine safe surgery and follow-up. Parental support and a committed referring psychiatrist strengthen the care plan; they cannot substitute for the patient's own consent.

Choosing a Circuit Target
-------------------------

### Anatomical evidence versus regulatory indication

OCD involves dysfunctional communication across cortico-striato-thalamo-cortical circuits. Implanted DBS electrodes deliver programmable electrical stimulation within these circuits; the clinical effect depends on the engaged pathways and settings, not simply on placing an electrode near an obsession-related region. Transcranial stimulation does not require intracerebral leads. [\[2\]](#cite-2 "Reference [2]")

The US distinction matters in an oral examination. The FDA humanitarian device exemption specifies **bilateral anterior limb of the internal capsule** stimulation as an adjunct to medication for adults with severe, treatment-resistant OCD who have failed at least three SSRIs. Meanwhile, Congress of Neurological Surgeons guidance supports bilateral subthalamic nucleus DBS and identifies bilateral nucleus accumbens or bed nucleus of the stria terminalis as additional evidence-supported options. The guideline does not establish one universally superior target. [\[3\]](#cite-3 "Reference [3]")

Target choice therefore belongs to an experienced multidisciplinary program. The team should discuss its own expertise, the planned device and indication, expected programming burden, and how outcomes will be measured.

### What benefit—and burden—should consent describe?

A 2022 meta-analysis reported a 66% full-response rate at last follow-up, defining response as at least a 35% Y-BOCS reduction. This is a pooled study result, **not an individual prognosis**. Depressive symptoms may improve, but neither remission nor restored independence is guaranteed; social participation and daily functioning deserve separate follow-up measures. [\[4\]](#cite-4 "Reference [4]")

Unlike stereotactic lesioning, DBS does not intentionally create a permanent therapeutic lesion: stimulation can be adjusted or stopped. It is not consequence-free or fully reversible, however. Implantation carries surgical risks, while ongoing treatment entails programming visits, device maintenance, and possible generator replacement. Stimulation-related mood changes also require monitoring. [\[5\]](#cite-5 "Reference [5]")

> **Clinical pearl:** In OCD DBS, apparent psychiatric relapse can be a device problem. Sudden worsening after prior benefit warrants an immediate safety assessment *and* device interrogation. [\[5\]](#cite-5 "Reference [5]")

Consent and Longitudinal Care
-----------------------------

### Assess capacity for this decision

Severe OCD or depression does not, by itself, establish incapacity. In a private interview, ask him to explain the proposed operation and demonstrate four abilities: understanding its risks and alternatives; appreciating how those consequences apply to him; reasoning about trade-offs; and expressing a voluntary, consistent choice. Probe whether unrealistic expectations or family pressure distort his decision, and document the assessment. [\[6\]](#cite-6 "Reference [6]")

### If symptoms abruptly return

Suppose he improves after implantation, then arrives three months later with severe depression, suicidal ideation, and renewed rituals. Treat suicidality as an emergency while asking the DBS team to check stimulation status, battery or charging state, settings, and hardware. Interrupted stimulation, including battery depletion, has been associated with abrupt mood and OCD worsening; it is a leading device-related possibility, not a diagnosis to assume before interrogation. [\[5\]](#cite-5 "Reference [5]")

After stabilization, sustained care requires:

1. Scheduled specialist programming and a clear route for urgent device assessment.
2. Psychiatric follow-up for medication review, mood monitoring, and suicide-risk reassessment.
3. Renewed ERP to address residual rituals and translate symptom gains into daily activity.
4. Family involvement—with the patient's permission—to support appointments without reinforcing compulsions.

Key Points for Board Exams
--------------------------

- Verify genuinely adequate pharmacotherapy and ERP before labeling OCD treatment-refractory.
- Distinguish the FDA-authorized capsule indication from other evidence-supported DBS targets.
- Quote response rates as cohort estimates, not promises; track function as well as Y-BOCS.
- Assess decision-specific capacity even when family members provide extensive support.
- In sudden postoperative deterioration, manage suicide risk while urgently evaluating stimulation interruption.

DBS is best framed as a long-term, multidisciplinary treatment rather than a single operation. In this case, candidacy is plausible; the decisive work is careful consent, target planning, and a credible plan for the years after implantation.

Frequently Asked Questions
----------------------------

### Does transient improvement with ECT argue against DBS for this patient's OCD?

No. His mood improved transiently, but his OCD did not. ECT response is not a standard prerequisite for OCD DBS candidacy.

### Which OCD DBS target is specified in the US humanitarian device exemption?

Bilateral stimulation of the anterior limb of the internal capsule, as an adjunct to medication, is the specified indication. [\[3\]](#cite-3 "Reference [3]")

### Can this patient consent if he has severe depression?

Potentially. Assess whether he can understand, appreciate, reason about, and voluntarily communicate this specific treatment decision; diagnosis alone does not determine capacity. [\[7\]](#cite-7 "Reference [7]")

### What should be checked when depression and rituals worsen suddenly after DBS?

Address immediate suicide risk and urgently interrogate the device for interrupted stimulation, battery or charging problems, changed settings, or hardware failure. [\[5\]](#cite-5 "Reference [5]")
