For MCAI preparation, turn that gap into your revision agenda: practise explaining mechanisms, comparing alternatives and answering timed questions rather than rereading entire textbooks. **Scope: September 2026.** MCAI is the Membership examination, not Final FCAI. This plan concentrates on the MCQ gateway; passing it allows progression to the separate MCAI OSCE/SOE assessments, rather than awarding Membership by itself. [\[1\]](#cite-1 "Reference [1]") 1. Build Your Plan Around the Published Format ---------------------------------------------- Start by checking your sitting’s instructions. CAI’s current MCQ page announces replacement of the multiple true/false paper with a second single-best-answer paper: **two papers, each containing 50 SBAs in 90 minutes**. Older downloadable guides still describe the previous format, so resolve discrepancies with CAI before building mocks. Do not mistake a small anatomy or statistics allocation for permission to omit either. [\[2\]](#cite-2 "Reference [2]") Avoid importing historical pass marks, negative-marking assumptions or compensation rules from another examination. Read your current candidate instructions and ask CAI about any unresolved scoring question; an unofficial question-bank percentage should not become your personal pass threshold. 2. Convert the Syllabus Into Testable Tasks ------------------------------------------- Download the MCAI syllabus and give each topic one of three labels: ‘cannot explain’, ‘can explain’ or ‘can answer unseen questions’. Start with the first category, but require question performance before promoting a topic to the third. Use these syllabus-aligned tasks rather than vague goals such as ‘revise physiology’: - **Physiology:** draw a pressure–volume relationship, label its axes and explain what changes under different conditions. - **Pharmacology:** compare clearance, volume of distribution and half-life; distinguish pharmacokinetic from pharmacodynamic explanations. - **Physics and measurement:** explain a monitor’s operating principle, calibration and limitations. - **Anatomy:** draw relevant airway or regional anatomy and label neighbouring structures. - **Statistics:** choose an appropriate statistical test and distinguish type I from type II error. [\[3\]](#cite-3 "Reference [3]") ### Use Clinical Work as a Prompt After encountering a vaporiser, neuromuscular monitor or unfamiliar drug, write one basic-science question to investigate later. Keep the task narrow: ‘Explain this device’s measurement principle’ is more useful for this session than ‘Read about monitoring’. > **Pro Tip:** For each equation, practise naming the variables, checking units and stating its assumptions before inserting numbers. 3. Review Errors Until You Can Explain the Distractors ------------------------------------------------------ Use one syllabus-matched SBA bank as your main practice resource. Choose questions with referenced explanations and subject filters; use a basic-science textbook to resolve specific gaps rather than beginning another cover-to-cover reading project. For each incorrect answer—and each lucky guess—follow this sequence: 1. Identify the precise concept being tested. 2. Explain why your selected option was less appropriate. 3. Explain why the best competing distractor fails. 4. Record one correction under ‘knowledge’, ‘reasoning’, ‘units’ or ‘stem-reading’. 5. Retest the concept with a different question. Schedule short retrieval sessions after a few days and again the following week. Spaced repetition has improved retention and knowledge transfer in physician research, although this does not establish an MCAI-specific pass benefit or an ideal revision interval. [\[4\]](#cite-4 "Reference [4]") ### Make Group Study Earn Its Time Ask each colleague to bring two disputed SBAs and a source-supported explanation. Limit discussion to the decisive mechanism, then have everyone answer a related question independently. For later SOE preparation, add five-minute spoken explanations with a colleague asking follow-up questions. CAI describes two 30-minute SOEs, each comprising six five-minute questions that explore understanding as well as knowledge; practise explaining rather than reciting lists. [\[5\]](#cite-5 "Reference [5]") 4. Rehearse the Pace Around Your Rota ------------------------------------- For the published 50-question, 90-minute paper, you have an average of 108 seconds per question. Try completing your first pass within 75 minutes, leaving 15 minutes for review; this is a suggested pacing strategy, not a College rule. [\[2\]](#cite-2 "Reference [2]") On each SBA, identify the requested outcome before examining the options. If you become stuck, make a provisional choice and return later if the platform permits; change an answer because you identified a reasoning error, not simply because it feels uncomfortable. ### Study Schedule Template Use this eight-week framework as a starting point, extending it if your baseline or rota requires. During heavy on-call weeks, retain short error-retrieval sessions and move the full mock rather than rushing it without review. Complete the official online trial using your intended device and location. Check current identification, equipment and permitted-material requirements rather than assuming that your usual study setup is acceptable. [\[6\]](#cite-6 "Reference [6]") Common Pitfalls --------------- - **Rehearsing an outdated paper:** verify the format before choosing practice materials. - **Neglecting equipment:** schedule it explicitly alongside pharmacology and physiology. - **Counting remembered answers as progress:** track unseen questions separately. - **Collecting courses without a goal:** choose feedback on a documented weakness, not another broad content overview.
