  Most FCPS Part II candidates do not fail because they are lazy. They fail because they prepare for Obstetrics &amp; Gynaecology as if it were just a larger FCPS Part I: too much passive reading, too little bedside rehearsal, and a dangerous delay in sorting exam eligibility paperwork. For FCPS Part II major subjects, you need a theory pass and then a clinical performance that stands up under direct observation. CPSP’s current Obstetrics &amp; Gynaecology clinical guidance describes two long cases, one obstetric and one gynaecologic, plus TOACS, with an aggregate 60% pass mark; CPSP also states that clinical components are scored independently by paired examiners. As of July 2026, CPSP lists FCPS-II major-subject theory sittings on February 16, 2026 and September 29, 2026. That means your preparation should be built backward from a real date, not from a vague plan to 'revise more later.' [\[2\]](#cite-2 "Reference [2]")

Study the exam you will actually sit
------------------------------------

For major subjects in Pakistan, CPSP’s current guidance still points candidates to an online theory format, while the clinical side for Obstetrics &amp; Gynaecology is a separate performance test with timed long cases and TOACS stations. So your weekly plan must always include both screen-based question practice and spoken clinical rehearsal. [\[3\]](#cite-3 "Reference [3]")

Build your preparation around three parallel tracks:

- **Theory:** timed online MCQ blocks, followed by review of wrong answers only.
- **Clinical:** one obstetric long case, one gynaecology long case, and a TOACS circuit every week.
- **Eligibility:** research, article, workshop, and portal checklist review every two weeks.

CPSP’s research pages now make synopsis approval part of the pathway for FCPS-II candidates, and they also state that a synopsis-based published article is required for first-fellowship candidates appearing in FCPS Part II theory. Do not let administrative requirements become the reason a well-prepared candidate misses the sitting. [\[4\]](#cite-4 "Reference [4]")

> **Pro Tip:** If you are 10 to 12 weeks from the exam, never end a study day with reading alone. Finish with either 20 timed questions or one spoken case presentation.

Prioritize cases that generate discussion
-----------------------------------------

In FCPS Part II, broad discussion matters more than obscure facts. Prioritize topics that let you demonstrate diagnosis, risk assessment, investigations, immediate management, definitive management, and counselling in one answer.

A practical high-yield list for long cases and TOACS is:

- Severe pre-eclampsia/eclampsia, APH, PPH, labour dystocia, CTG interpretation, induction, previous cesarean and VBAC counselling.
- Preterm labour, PPROM, diabetes or cardiac disease in pregnancy, sepsis, and early pregnancy complications.
- Abnormal uterine bleeding, fibroid uterus, adnexal mass, infertility, prolapse, postmenopausal bleeding, and core gynae-oncology red flags.

For each topic, make a one-page case sheet with the same headings every time: likely diagnosis, key differentials, must-not-miss complication, first-line investigations, immediate management, definitive plan, and counselling points. Repeating those sheets in spaced cycles is far more efficient than rereading full chapters, and medical-education reviews support spaced repetition plus deliberate clinical practice over passive review. -----------------------

If you are aiming for the September 29, 2026 sitting, use a 12-week countdown. Keep the structure simple enough to survive calls, labour room duties, and post-call fatigue. [\[2\]](#cite-2 "Reference [2]")

### Resource mix that actually works

Use resources by purpose, not by habit:

- **Question banks:** for timing, pattern recognition, and your error log.
- **Core textbooks:** to repair concepts only after you miss a question or fumble a case.
- **Unit protocols and national/international guidelines:** for thresholds, escalation, and route-of-delivery decisions.
- **Small study groups:** two to four serious candidates are enough for viva drilling.
- **Real patients and ward rounds:** best source for long-case flow, consent language, and examination polish.

Practice for what examiners score
---------------------------------

The long-case mark is not just about naming the diagnosis. CPSP’s current Obstetrics &amp; Gynaecology clinical instructions explicitly assess approach to the patient, bedside manners, informed consent, systematic examination, presentation, interpretation, investigations, and management discussion. [\[1\]](#cite-1 "Reference [1]")

Use this checklist in every practice case:

- Introduce yourself, confirm identity, take consent, and ask for appropriate exposure and chaperone.
- Give a 30-second summary before details.
- State a problem list, not a stream of findings.
- Rank investigations into urgent, confirmatory, and pre-operative.
- Present management in order: stabilize first, then definitive treatment, then counselling.
- End with follow-up and complication surveillance.

For TOACS, your method should be mechanical. Read the task first, identify whether it is examination, data interpretation, counselling, or emergency management, then give the final answer early and justify it briefly. In the current CPSP OBGYN clinical guidance, TOACS stations are timed at 4 to 8 minutes, so long introductions waste marks. [\[1\]](#cite-1 "Reference [1]")

> **Pro Tip:** In obstetric emergencies, say the maternal stabilization steps first. Examiners trust candidates who show safe sequencing.

---------------

Avoid these repeat mistakes:

- Reading whole chapters without doing same-day questions.
- Practising examination silently instead of presenting aloud.
- Giving generic management without thresholds, timing, or counselling.
- Leaving TOACS until after theory results.
