They know the medicine, but they have not practised showing the exact domains RCEM examiners score in an eight-minute station. That matters because the FRCEM OSCE is a 16-station, domain-marked exam with one minute of reading time before each station, two rest stations, and a requirement to pass at least one of the three resuscitation stations overall. RCEM uses domain-based marking, with one to four weighted domains tested in each station, and the station briefing tells you the task, the role of the examiner, and the domains being assessed in a pie chart. If you ignore that pie chart and default to a full history or a polished speech, you can look busy but still miss marks. The briefing sheet is telling you where the marks are. RCEM publishes the FRCEM OSCE blueprint: 3 complex stable patient stations, 2 SLO 7/8 stations, 1 supervision/teaching station, 3 resus stations, 2 injured patient stations, 1 PEM station, 2 management stations, and 2 management-or-research stations. FRCEM is testing consultant-level behaviour: prioritising risk, leading others, resolving conflict, and making workable plans under pressure, not just recalling guidelines. Rehearse Stations the Way the Exam Runs --------------------------------------- Practise in true FRCEM format: one minute to read, eight minutes to perform, no visible countdown timer, and only an analogue clock in the room. There are no double stations, so you need to get good at switching tasks fast and resetting mentally after each station. [\[5\]](#cite-5 "Reference [5]") After every mock, get your partner or consultant to score only the domains that mattered in that station. Ask for three things: what to keep, what to drop, and what exact phrase or behaviour would have moved you up one level. That is much more useful than vague feedback like “be more confident.” ### A Simple Eight-Minute Station Template 1. **0:00-0:30** — open clearly, identify the problem, show leadership. 2. **0:30-5:30** — gather only the information needed for the task. 3. **5:30-7:00** — commit to a plan, prioritise, escalate appropriately. 4. **7:00-8:00** — summarise, address risk, confirm next steps. > **Pro tip:** Record one mock per week. FRCEM candidates often discover that they sound less structured than they feel. Treat the CLA and Management Stations as Easy Marks to Train ------------------------------------------------------------ For the FRCEM OSCE, RCEM sends the critical appraisal journal article four weeks before the exam. The CLA station is a guided conversation about internal validity, external validity, and clinical applicability; you do not need to memorise the paper and there is no expectation to perform calculations in the station. [\[2\]](#cite-2 "Reference [2]") Make a one-page CLA sheet for the article: - Research question and design - Biggest threats to validity or bias - Main results in plain English - Whether the results apply to your ED patients - What would change, and what would not change, in practice Management and research stations are often underprepared. Use local complaint pathways, incident investigation tools, governance processes, and patient-flow problems to practise giving a consultant-level answer: immediate safety, communication with stakeholders, documentation, escalation, and what happens after the shift. The blueprint specifically includes management and management-or-research stations, so ring-fence these in your study plan. [\[3\]](#cite-3 "Reference [3]") Use the Right Resources in the Right Way ---------------------------------------- The best FRCEM OSCE resources are the official regulations and blueprint, the RCEM curriculum SLO pages, domain-marking guidance, RCEM learning materials, and a small study group that will actually score you. Use textbooks and guidelines to fix knowledge gaps, but use mock stations to convert knowledge into performance. [\[3\]](#cite-3 "Reference [3]") A good three-person study group works well: - **Candidate:** runs the station. - **Examiner:** scores domains only. - **Observer:** watches timing, structure, and missed opportunities. Rotate roles every station. That exposes you to more cases and teaches you what examiners notice. Common Pitfalls That Cost Good Candidates a Pass ------------------------------------------------ The big mistakes are predictable. Candidates prepare like the SBA, over-talk in practical stations, neglect resus because they hope to compensate elsewhere, or leave CLA and management until the final week. That is especially risky in an exam where you must pass at least one resus station as well as clear the overall pass mark. [\[6\]](#cite-6 "Reference [6]") Also, do not ignore admin. The FRCEM OSCE is delivered in London, late candidates are not admitted after the registration window closes, and equipment is provided if needed. Plan travel early so your preparation is protecting your exam day, not sabotaging it.
