A 37-year-old with a BRCA1 pathogenic variant asks whether a normal CA-125 means she can postpone surgery. Across the hall, a patient newly diagnosed with endometrial cancer has no notable family history. Both encounters test the same skill: **do not let an apparently reassuring finding replace a hereditary-risk assessment**.

Recognize the syndrome before choosing the operation
----------------------------------------------------

### BRCA1/2: think tubes and ovaries, not ovaries alone

Hereditary breast and ovarian cancer (HBOC) associated with BRCA1/2 increases breast cancer risk and the risk of cancers classified as ovarian, fallopian tube, or primary peritoneal. BRCA1-associated tubo-ovarian cancer generally occurs earlier than BRCA2-associated disease; that difference drives surgical timing. Remember the fallopian tube: high-grade serous cancer can arise from its fimbrial epithelium. [\[1\]](#cite-1 "Reference [1]")

Refer patients with epithelial ovarian, fallopian tube, or primary peritoneal cancer for germline genetic evaluation regardless of family history. In an unaffected patient, ask about cancer on **both** parental sides, including ovarian cancer, early breast cancer, male breast cancer, and relevant pancreatic or prostate cancer. A small family or few older female relatives can conceal inherited risk. [\[2\]](#cite-2 "Reference [2]")

### Lynch syndrome: the endometrium may be the warning

Lynch syndrome results from pathogenic variants affecting mismatch repair—MLH1, MSH2, MSH6, or PMS2—or EPCAM deletions that silence MSH2. Its gynecologic signal is endometrial cancer, with ovarian cancer risk that varies substantially by gene. Do not assume every Lynch carrier has the same indication for oophorectomy. [\[3\]](#cite-3 "Reference [3]")

Every newly diagnosed endometrial cancer deserves systematic Lynch assessment; tumor mismatch-repair immunohistochemistry or microsatellite-instability testing can guide further evaluation. Loss of MLH1/PMS2 should prompt tumor MLH1 promoter methylation testing because sporadic endometrial tumors can also be mismatch-repair deficient. An abnormal tumor screen raises suspicion; **germline testing establishes inherited risk** and enables testing of relatives. [\[2\]](#cite-2 "Reference [2]")

> **Clinical Pearl:** A normal CA-125 or transvaginal ultrasound does not make it safe to defer indicated BRCA risk-reducing surgery. These tests have not reliably detected early-stage ovarian cancer in high-risk patients. [\[1\]](#cite-1 "Reference [1]")

Time surgery to the gene and the patient
----------------------------------------

### For BRCA, remove both tubes and ovaries

Recommend risk-reducing bilateral salpingo-oophorectomy (RRSO) after childbearing, generally at **35–40 years for BRCA1** and **40–45 years for BRCA2**. Discuss earlier affected relatives and individual circumstances rather than treating birthdays as rigid deadlines. RRSO substantially reduces tubo-ovarian cancer risk, but a small primary peritoneal cancer risk remains. [\[4\]](#cite-4 "Reference [4]")

Do not present salpingectomy with delayed oophorectomy as equivalent protection: its cancer-prevention effectiveness remains unproven, and specialist discussion or a clinical trial is appropriate. Likewise, BRCA status alone does not automatically require hysterectomy. If RRSO is planned, ensure the surgical and pathology teams know the hereditary indication so both tubes and ovaries receive thorough examination for occult disease. [\[4\]](#cite-4 "Reference [4]")

### For Lynch, distinguish hysterectomy from oophorectomy

After childbearing, discuss risk-reducing **total hysterectomy** to prevent endometrial cancer. Discuss bilateral salpingo-oophorectomy separately: ovarian risk, age, menopausal status, family history, and the specific Lynch gene matter. In particular, evidence has not established a significantly increased ovarian cancer risk for PMS2 carriers; reflexively removing a young patient’s ovaries may impose harm without a clear corresponding benefit. [\[3\]](#cite-3 "Reference [3]")

There is no single operative age that fits every Lynch carrier. Make the plan with gynecologic oncology and genetics input, and revisit it as reproductive goals change. A hysterectomy prevents uterine cancer; it does **not** replace Lynch-associated colorectal surveillance. [\[3\]](#cite-3 "Reference [3]")

Counsel for the years before and after surgery
----------------------------------------------

When a patient defers Lynch surgery, teach them to report abnormal uterine or postmenopausal bleeding promptly and evaluate symptoms with endometrial sampling. Endometrial biopsy every one to two years from approximately age 30–35 may be considered, but evidence that surveillance improves survival is limited. Do not describe ultrasound or biopsy as a proven substitute for risk-reducing surgery. [\[3\]](#cite-3 "Reference [3]")

Before premenopausal oophorectomy, discuss infertility, immediate menopause, sexual symptoms, and long-term bone and cardiovascular health. Discuss menopausal hormone therapy individually; it may be appropriate for a carrier without a personal breast cancer history, while prior breast cancer changes the risk–benefit conversation. Arrange follow-up rather than ending counseling when the pathology report is benign. [\[5\]](#cite-5 "Reference [5]")

Finally, explain test results precisely. A variant of uncertain significance is **not** a pathogenic variant and should not, by itself, trigger preventive surgery. Once a familial pathogenic variant is confirmed, offer relatives targeted cascade testing through appropriate genetic services. [\[6\]](#cite-6 "Reference [6]")

Key Takeaways
-------------

- **BRCA1:** Discuss RRSO after childbearing, generally at 35–40; **BRCA2:** generally at 40–45. [\[4\]](#cite-4 "Reference [4]")
- **Lynch:** Assess every endometrial cancer systematically; tailor hysterectomy and oophorectomy counseling to the gene and patient. [\[2\]](#cite-2 "Reference [2]")
- Neither reassuring ovarian screening nor a variant of uncertain significance is a sound basis for preventive-surgery decisions. [\[1\]](#cite-1 "Reference [1]")

The board-exam distinction is also the clinical one: BRCA prevention centers on timely removal of tubes and ovaries; Lynch prevention centers on the endometrium, with ovarian surgery individualized. Name the gene, establish the patient’s goals, then make the prevention plan.

Frequently Asked Questions
----------------------------

### Does a BRCA pathogenic variant mean the uterus should be removed during RRSO?

No. BRCA status alone does not automatically require hysterectomy; discuss separate uterine indications and the patient’s preferences.

### Can CA-125 and ultrasound safely replace RRSO for a BRCA carrier?

No. They have not reliably detected early-stage ovarian cancer in high-risk patients and are not equivalent to RRSO.

### Does mismatch-repair loss in an endometrial tumor prove Lynch syndrome?

No. It prompts further evaluation; somatic causes, including MLH1 promoter methylation, occur. Germline evaluation establishes inherited risk.

### Must every premenopausal Lynch carrier have both ovaries removed with hysterectomy?

No. Oophorectomy should be individualized by gene-specific ovarian risk, age, menopausal effects, family history, and patient preferences.
