Many MRCEM Primary candidates revise the Emergency Medicine cases they see at work, then discover their practice scores are weakest in anatomy or physiology. **This is a basic sciences exam**, not a rehearsal for the later clinical MRCEM SBA or OSCE. Start with the RCEM blueprint, then make every question session expose a gap you can fix. [\[1\]](#cite-1 "Reference [1]")

Build Your Plan Around the Blueprint
------------------------------------

The MRCEM Primary is one three-hour, computer-based paper with 180 single-best-answer questions: an average of **one minute per question**. RCEM’s October 2026 information pack gives indicative allocations of 59 anatomy, 58 physiology, 30 pharmacology, 15 microbiology, nine pathology and nine evidence-based medicine questions; live totals may vary slightly. Use those numbers to allocate study time, not to predict your paper. [\[1\]](#cite-1 "Reference [1]")

### Put the largest categories first

- **Anatomy:** Work through limb, thoracic, abdominal, head-and-neck and nervous-system anatomy. Test yourself on structures, relationships and consequences of injury rather than merely recognising diagrams.
- **Physiology:** Explain cardiovascular, respiratory, renal and endocrine mechanisms aloud. If you cannot predict what happens when a variable changes, revisit the mechanism before trying more questions.
- **Pharmacology:** Compare drug classes by mechanism, effects, adverse reactions and safety considerations. Include the poisoning and emergency-drug topics shown in the current blueprint.
- **Smaller categories:** Give microbiology, pathology and evidence-based medicine recurring sessions. Nine questions are still nine opportunities you should not abandon. [\[1\]](#cite-1 "Reference [1]")

Check which syllabus applies to **your sitting** before collecting resources. RCEM said the October 27, 2026 sitting used the existing Basic Sciences Syllabus; its updated syllabus becomes the official reference from the April 21, 2027 sitting. If you are preparing for 2027, check RCEM’s published syllabus and pack rather than relying on an older question bank’s topic list. [\[2\]](#cite-2 "Reference [2]")

Use a 12-Week Study Schedule Template
-------------------------------------

Treat this as a template, not a demand for daily study after shifts. Reserve four focused sessions and one short review session each week; move sessions around your rota while keeping all six blueprint categories in view.

WeeksMain taskWeekly check1–2Map the syllabus; take a mixed baseline setRecord errors by category and subtopic3–6Cover anatomy and physiology systematicallyAdd pharmacology twice weekly; revisit errors7–9Complete pharmacology and smaller categoriesDo timed, mixed question sets10–11Sit full 180-question timed mocksReview missed items and time lost12Repair persistent gaps; repeat pacing practiceStop adding resources; check exam instructions

For a 45-minute weekday session, try 15 minutes retrieving yesterday’s weak facts, 20 minutes answering syllabus-matched questions and 10 minutes explaining errors. On a longer day off, review a connected block—such as renal physiology and fluid pharmacology—then test it without notes. Practice testing and spreading sessions over time have stronger evidence for learning than relying on rereading alone. [\[3\]](#cite-3 "Reference [3]")

> **Pro Tip:** Protect your first anatomy and physiology blocks in the rota. If shifts disrupt a week, shorten question sets rather than dropping a whole category until the final fortnight.

Make Resources Earn Their Place
-------------------------------

Use the **RCEM syllabus and blueprint as your checklist**, a basic sciences textbook or trusted teaching notes to repair gaps, and a Primary-style question bank to test retrieval. Start with RCEM sample questions to inspect the wording; do not assume every commercially written item matches the official scope or difficulty. RCEM describes an SBA as a question where several options may be plausible but only one is fully correct. [\[1\]](#cite-1 "Reference [1]")

After each question set, log three things: the subtopic, why your chosen answer was tempting, and the fact or mechanism that distinguishes the best answer. Reattempt missed questions several days later without viewing explanations first. If the error persists, return to the relevant syllabus heading and a concise reference rather than doing another large set blindly.

Use a study group for **short explanation rounds**, not shared passive reading. Ask a colleague to justify why one nerve lesion fits a finding while the distractors do not, or to explain how a physiological change affects a measured value. Keep a running list of disagreements to check against reliable sources after the session.

Rehearse the Actual Three-Hour Paper
------------------------------------

Practise on a screen with 180 questions and a three-hour timer at least twice before your sitting. Use one-minute-per-question as your overall budget: aim to reach question 60 near 60 minutes and question 120 near two hours, adjusting for your interface and any approved adjustments. A difficult item should not consume the time you need for several straightforward ones. [\[1\]](#cite-1 "Reference [1]")

1. Read the final question and identify the basic science concept being tested.
2. Eliminate options that conflict with that concept; choose the single best remaining answer.
3. If uncertain, make your best selection, flag it if the interface permits and move on.
4. Revisit flagged items only after reaching the end.

RCEM awards one mark for a correct answer, zero for an incorrect answer and applies **no negative marking**. Its pass mark is set by the Angoff method, so avoid chasing an assumed fixed pass percentage—and do not leave a question unanswered simply because you are uncertain. Confirm the available review controls in your test instructions before relying on a flagging strategy. [\[4\]](#cite-4 "Reference [4]")

Common Pitfalls to Avoid
------------------------

- **Revising for the next exam instead:** Clinical management discussions matter in practice, but prioritise the Primary’s basic sciences blueprint over MRCEM SBA-style case management. [\[5\]](#cite-5 "Reference [5]")
- **Counting questions without reviewing them:** Track whether repeat mistakes concern knowledge, interpretation or pacing; give each a different remedy.
- **Reading outdated syllabus lists:** Recheck RCEM material for your sitting, particularly if you plan to sit from April 2027 onwards. [\[2\]](#cite-2 "Reference [2]")
- **Never practising a full paper:** A short question set cannot show whether your pace holds for 180 decisions.

Key Takeaways: Act This Week
----------------------------

- Download the RCEM information pack and identify the syllabus for your sitting.
- Put one anatomy, one physiology and one pharmacology block in your rota this week.
- Complete a timed mixed set; label every error by blueprint subtopic.
- Book two full-length mock slots before your final revision week.

You do not need a perfect rota to prepare well. Make the next session specific: choose a blueprint heading, test what you know, repair one weakness and return to it later.

Frequently Asked Questions
----------------------------

### How much time should I allow per MRCEM Primary question?

The paper has 180 questions in three hours, averaging one minute each. Practise that overall pace, moving on from difficult items rather than enforcing a strict minute on every question. [\[1\]](#cite-1 "Reference [1]")

### Which MRCEM Primary subjects should I revise first?

Start with anatomy and physiology, the two largest categories in RCEM’s indicative blueprint. Keep regular sessions for pharmacology and the smaller categories rather than leaving them until the end. [\[1\]](#cite-1 "Reference [1]")

### Should I guess if I cannot work out an answer?

Yes. RCEM does not apply negative marking to MRCEM Primary, so select your best answer rather than leave an item blank. [\[4\]](#cite-4 "Reference [4]")

### Which basic sciences syllabus applies to an April 2027 sitting?

RCEM says its updated Basic Sciences Syllabus becomes the official reference from the April 21, 2027 examination onwards. Check the current RCEM syllabus and information pack when you prepare. [\[2\]](#cite-2 "Reference [2]")
