You finish a shift having managed several difficult patients, yet a practice MRCEM SBA question still leaves you choosing between two plausible answers. That is the preparation trap: clinical exposure gives you cases, but it does not automatically train you to identify the **single best answer** from the details supplied. Start turning those cases into deliberate, timed decisions.

As of November 2026, the MRCEM SBA comprises **180 questions: 90 in each of two two-hour papers**, with a one-hour break. It tests the range of emergency department presentations and follows the MRCEM Primary; unlike the subsequent OSCE, it does not assess your performance in simulated stations. RCEM delivers its theory exams by computer-based testing through Surpass Assessment. [\[1\]](#cite-1 "Reference [1]")

1. Let the MRCEM SBA blueprint set your priorities
--------------------------------------------------

Download RCEM’s MRCEM SBA regulations and information pack before opening a textbook. Its published blueprint allocates 55 questions to complex stable patients, 40 to resuscitation, 30 to injured patients, 25 to paediatric emergency medicine, 20 to procedural skills and basic anaesthetic care, and 10 to complex or challenging situations. Use those allocations to plan coverage, not to predict individual questions. [\[2\]](#cite-2 "Reference [2]")

### Build a coverage tracker

Make six rows matching those blueprint areas. Under each, list the relevant Year 1–3 Specialty Learning Outcomes (SLOs), then mark topics **untested**, **weak** or **ready to revisit** after practice questions. Give the larger areas more sessions, but reserve time for the smaller areas: missing safeguarding or procedural questions because you never studied them is avoidable.

- For complex stable patients, rotate through presentations such as chest pain, headache, abdominal pain and poisoning; practise distinguishing likely diagnoses from the next appropriate action.
- For resuscitation and injury, test priorities in the order a case presents them: immediate threat, first intervention, reassessment and disposition.
- For paediatrics, use separate question sets rather than assuming adult answers transfer to children.

> **Pro Tip:** When two options both sound reasonable, ask, “Which one answers the precise question at this point in the patient’s ED journey?” RCEM describes SBA options as potentially plausible, with only one fully correct answer. [\[3\]](#cite-3 "Reference [3]")

2. Make questions your revision engine
--------------------------------------

Use a question bank that lets you select MRCEM SBA-style clinical topics and run timed, mixed blocks. Begin a session with 20–25 questions **without notes**. For each missed or guessed answer, record the clue you overlooked, why your choice was less appropriate, and one rule you will apply to the next similar presentation.

Keep that error log short enough to review after a shift. For example: “I chose the diagnosis before checking whether the stem asked for investigation or immediate management.” Retest logged topics several days later, then mix them with other systems; practice testing and spaced review are better-supported learning approaches than repeated passive reading. [\[4\]](#cite-4 "Reference [4]")

Use resources for distinct jobs, rather than reading everything cover to cover:

- **RCEM blueprint and sample questions:** Check scope, wording and image-based question style first. RCEM’s samples include photographs, radiographs and investigation data. [\[2\]](#cite-2 "Reference [2]")
- **Emergency medicine textbook:** Read a focused section only when your error log exposes a knowledge gap.
- **Current UK clinical guidance:** Check a disputed management answer against the relevant authoritative guidance, noting when its recommendations apply.
- **Study group or course:** Bring three questions where the *reasoning* remains unclear; ask colleagues to defend competing options using the stem, not recall alone.

3. Use an eight-week study schedule template
--------------------------------------------

Fit this plan around your rota rather than promising daily marathon sessions. On working days, aim for one 25-question block plus review when feasible; on a day off, use a longer mixed set. If you have fewer than eight weeks, retain the sequence but shorten the topic blocks—not the full-paper rehearsals.

WhenMain taskCheckpointWeeks 1–2Map the blueprint; test complex stable presentations and resuscitationStart an error log and identify your weakest SLOsWeeks 3–4Cover injury, paediatrics, procedures and challenging situationsComplete timed topic sets across all six areasWeeks 5–6Switch to mixed 90-question papers; revisit recurring errorsReview decisions and timing, not just percentage correctWeeks 7–8Rehearse both papers with the one-hour break; target remaining gapsComplete at least one full 180-question simulation

After each block, schedule the next session by what you missed, not by which chapter comes next. If a run of shifts disrupts the plan, restart with a mixed block and your error log; you do not need to reread every topic to regain momentum.

4. Rehearse the actual two-paper decision pace
----------------------------------------------

Two hours for 90 questions gives you an **average of 80 seconds per question**. In a timed practice paper, aim to reach question 45 at roughly the one-hour mark. If a stem is consuming disproportionate time, choose your current best option, flag it if the practice interface permits, and move on; rehearse the review process available in your actual exam instructions. [\[1\]](#cite-1 "Reference [1]")

For every stem, identify the task before studying the options: diagnosis, interpretation, next investigation or management. Then extract the discriminating details—age, observations, time course, examination findings and available results. Change an answer during review only when you can point to evidence you missed, not because the first choice feels uncomfortable.

RCEM awards one mark for a correct MRCEM SBA answer, zero for an incorrect answer and applies **no negative marking**. Select an answer for every question, even if you must make an informed guess; do not plan around a fixed pass percentage, because RCEM uses standard setting to determine the pass mark. During the hour between papers, leave paper one behind and follow the break arrangements given for your sitting. [\[1\]](#cite-1 "Reference [1]")

Common pitfalls to avoid
------------------------

- **Revising as though this were the Primary:** Shift from isolated basic-science recall to applying knowledge to ED presentations and decisions.
- **Studying only familiar adult cases:** Give paediatrics and the smaller blueprint areas their own question sets. [\[2\]](#cite-2 "Reference [2]")
- **Counting questions without reviewing them:** A completed bank tells you little unless you can explain why the distractors lost.
- **Using old delivery instructions:** RCEM states that Surpass Assessment has delivered its theory exams since January 2026; check your current booking and candidate instructions rather than relying on older PDFs. [\[3\]](#cite-3 "Reference [3]")

Key takeaways: your actions this week
-------------------------------------

- Download the RCEM blueprint and label its six areas in a coverage tracker.
- Complete one timed, mixed 25-question baseline block and log every missed or guessed decision.
- Book two full-paper rehearsal slots in your rota and plan to practise the break.
- Choose your next topic from the error log, then verify disputed answers in authoritative guidance.

Start with the next decision
----------------------------

You do not need to master every presentation before attempting questions. Start with the baseline block, use its mistakes to direct the next session, and keep practising the decisions this particular exam asks you to make.

Frequently Asked Questions
----------------------------

### How many questions should I practise in one MRCEM SBA sitting?

Start with manageable 20–25-question blocks that you can review thoroughly. Later, practise timed 90-question papers and at least one full two-paper simulation.

### Should I leave an MRCEM SBA question unanswered if I am unsure?

No. RCEM applies no negative marking, so select your best answer for every question before time expires.

### Does preparing for the MRCEM Primary cover the MRCEM SBA?

No. The SBA requires application of clinical knowledge across ED presentations. Use its Year 1–3 SLOs and blueprint to identify what additional preparation you need.

### What should I do with a low score on a practice paper?

Sort missed and guessed questions by blueprint area and error type. Revise the recurring decision gaps, then retest them in a mixed timed block.
