They read extensively, yet struggle to examine fluently, identify signs accurately, or complete management discussions before the bell. Your preparation must therefore involve repeated, observed performance—not passive revision. As of August 2026, the current examination remains a ten-station circuit: four Short Clinical stations, one Development station, one History station, two Communication stations, and two Video stations. Short Clinical, Communication, and Video stations last nine minutes; History and Development each last 22 minutes. RCPCH is piloting a revised format, but it has not yet replaced the current circuit. After every practice station, ask your observer to record whether you: - used a focused, systematic approach; - adapted to the child and sought permission; - identified signs without inventing findings; - produced a prioritised differential diagnosis; - proposed safe investigations, management, referral, and escalation; - addressed the parent’s or young person’s concerns; - checked understanding and agreed next steps. > **Pro Tip:** Ask for domain-based feedback rather than “Did I pass?” A comment such as “your examination was systematic, but your interpretation was vague” gives you a precise target for the next attempt. Make Bedside Practice Your Core Resource ---------------------------------------- Four Short Clinical stations test real-patient examination, sign recognition, interpretation, and management. The system may include cardiovascular, respiratory, abdominal, neurological, musculoskeletal, ophthalmological, dermatological, growth, nutritional, renal, or syndromic presentations. [\[2\]](#cite-2 "Reference [2]") Practise at least four focused examinations each week. Use clinics and wards to seek children with stable signs, with consent and appropriate supervision. For every encounter, follow this sequence: 1. Complete the focused examination in approximately six minutes. 2. Summarise only the positive and important negative findings. 3. State your leading diagnosis and two credible alternatives. 4. Explain the significance of the findings. 5. Offer prioritised investigations and a safe management plan. Do not memorise theatrical examination routines. Examiners want a fluid approach adapted to the child’s age, cooperation, comfort, and clinical problem. Rehearse Each Scenario Station Differently ------------------------------------------ ### Development The Development station uses a scenario and requires a focused history followed by a verbal description of how you would assess selected developmental domains. You receive cues only when you describe the relevant assessment clearly, so state the activity, equipment, expected response, and what the result would mean. [\[2\]](#cite-2 "Reference [2]") Practise gross motor, fine motor and vision, speech and language, and social communication separately. Finish by estimating developmental level from demonstrated skills, identifying functional impact, and outlining multidisciplinary support. ### History and Communication For History stations, train yourself to define the presenting problem, chronology, red flags, functional impact, relevant background, family concerns, and safeguarding context within the allotted history period. Avoid performing a comprehensive clerking when the task requests a focused history. For Communication stations, use a simple structure: establish the agenda, assess existing understanding, explain in small sections, respond to emotion, check understanding, and agree next steps. Practise information giving, consent, critical incidents, ethics, education, and breaking difficult news—the examiner observes but does not rescue the conversation. [\[2\]](#cite-2 "Reference [2]") ### Video During the three-minute viewing period, observe systematically: appearance, consciousness, breathing pattern, colour, movement, symmetry, abnormal events, and audible signs. You must then describe signs, formulate a differential, and discuss initial management without replaying the clip. Common Pitfalls to Avoid ------------------------ - **Over-running the examination:** Leave time to summarise, interpret, and manage. - **Inventing signs:** State uncertainty and explain what you would do next. - **Giving unprioritised lists:** Name the most likely diagnosis and immediate actions first. - **Ignoring the stated task:** Rewrite the task in one sentence during reading time. - **Using jargon with families:** Translate terminology and check understanding. - **Missing escalation:** Explicitly state when senior, emergency, safeguarding, or multidisciplinary input is required. - **Treating feedback defensively:** Repeat the same station immediately using one corrected behaviour.
