You know how to manage a neurology consultation. The harder task is showing an examiner your reasoning before a short oral station ends. If your preparation still consists mainly of reading and written questions, start saying your decisions aloud: the applied examination tests how you work through a case, not just what you can recall. [\[1\]](#cite-1 "Reference [1]")

Know the station you are preparing for
--------------------------------------

The Royal College’s written 2026/applied 2027 neurology format describes **eight structured oral stations**, with the applied component taken after a pass in the written component. Its format table lists 15 minutes per station; general applied-exam guidance updated in October 2026 lists 14 station minutes, two additional reading minutes, and three minutes between stations for neurology. Use 14 minutes for speaking practice, but check your cohort-specific instructions closer to your examination rather than assuming either breakdown will apply unchanged. [\[1\]](#cite-1 "Reference [1]")

The applied examination uses station-specific global ratings, not the written paper’s short-answer marking approach. Ratings may consider focused history, examination, interpretation, clinical judgment, investigations, management, and communication; stations are equally weighted. Ask a practice examiner to assess those domains instead of counting how many facts you mentioned. [\[1\]](#cite-1 "Reference [1]")

Build a repeatable oral-case framework
--------------------------------------

Use a framework you can deliver without sounding scripted. In each mock case, make your priorities audible, then adapt when the examiner supplies new information.

1. **Define the problem:** Give a one-sentence summary with time course and neurological localization where appropriate.
2. **Identify urgency:** State the immediate safety concern and what you would do first, if one exists.
3. **Choose discriminators:** Request a few focused history or examination findings that distinguish your leading possibilities.
4. **Commit to a plan:** Give a prioritized differential, targeted investigations, initial management, and disposition or follow-up.
5. **Explain uncertainty:** State what finding would change your decision; avoid presenting an unconfirmed diagnosis as certain.

For example, when given acute weakness, first clarify onset and associated features, then explain what localization and urgent alternatives you are considering. Have your partner introduce an unexpected examination finding halfway through; practise revising your assessment aloud rather than defending your first answer.

> **Pro Tip:** Examiners may interrupt to probe or advance a station, and they are instructed to remain neutral. Practise responding to a redirect with a brief answer and an updated plan; do not treat a neutral expression as feedback. [\[1\]](#cite-1 "Reference [1]")

Rehearse the tasks, not just the diagnoses
------------------------------------------

The Royal College lists focused history, telephone consultation, interpretation of videos and investigations, examination findings, decision-making, and counselling among possible applied-station tasks. Rotate through these formats in your study group. Do not let eight consecutive diagnostic vignettes stand in for an eight-station rehearsal. [\[1\]](#cite-1 "Reference [1]")

- **Telephone consultation:** Have a colleague act as the referring clinician. Summarize the issue, ask for decisive missing information, give immediate actions, and specify when you need an update.
- **Images and tests:** Practise describing what you actually see before interpreting a scan, video, EEG report, or neurophysiology result. Then say how that interpretation changes management.
- **Examination:** Ask your partner for findings from a focused neurological examination. Explain why each requested component helps localization or alters your differential.
- **Counselling:** Have a partner play a patient asking about diagnosis, treatment, or prognosis. Explain your recommendation in plain language, acknowledge uncertainty, and check understanding.

Use the Royal College’s published sample station and competency documents to shape mock prompts. Use a current neurology textbook or question bank to repair a knowledge gap **after** a mock, not as a substitute for oral practice. Check condition-specific Canadian guidance when updating management notes; avoid memorizing a treatment script without its clinical indications. [\[1\]](#cite-1 "Reference [1]")

Study Schedule Template: six weeks
----------------------------------

If you have less time, preserve the weekly sequence and shorten the number of cases, not the feedback step. A 30-minute session can cover one timed station, a short critique, and one repeat attempt.

WhenOral practiceFeedback targetWeeks 1–2Three timed stations weekly across vascular, epilepsy, and systemic casesProblem summary, localization, immediate prioritiesWeeks 3–4Four stations weekly; add pediatric presentations, neuromuscular cases, and data interpretationTargeted questions, investigation choices, changing plansWeek 5Two mixed mini-circuits of four stationsPacing and performance across different tasksWeek 6One eight-station circuit, then brief repeats of weak formatsConsistency and clear handovers

Adapt the case mix to your adult or pediatric practice, but include unfamiliar presentations: the published applied content includes systemic and pediatric disorders for both candidate groups. After every station, record one effective decision, one missed discriminator, and one sentence you will say differently next time. Repeat that case within a few days. [\[1\]](#cite-1 "Reference [1]")

Common pitfalls to avoid
------------------------

- **Delivering a textbook chapter:** Lead with your likely diagnosis and its key alternative; expand only when asked.
- **Listing investigations without a decision:** Explain what a result would change in your management or disposition.
- **Ignoring communication:** Include a patient explanation or a concise consultant handover in practice, even when the case is medically complex.
- **Overreacting to one rough station:** Stations contribute equally to the overall applied score. Reset before the next encounter rather than replaying the last one. [\[1\]](#cite-1 "Reference [1]")

Key Takeaways: act this week
----------------------------

- Confirm your cohort’s applied-exam format and instructions on the Royal College site.
- Book two peers or supervisors to play examiners for three timed oral cases.
- Include one consultation, one interpretation task, and one counselling task.
- Keep a three-line feedback log and repeat your weakest case aloud.

Your goal is not a flawless speech. It is to make safe, focused neurological decisions visible to an examiner—then improve the next attempt using specific feedback.

Frequently Asked Questions
----------------------------

### Should I prepare for the neurology applied examination with written question banks?

Use them to identify knowledge gaps, then practise explaining the resulting clinical decisions aloud in timed stations. Written-answer practice alone does not rehearse oral reasoning or counselling.

### How long should I speak during a practice station?

Practise a 14-minute examiner interaction and check your cohort’s instructions closer to the exam. Published Royal College pages describe the station timing differently.

### What should I do if an examiner interrupts my answer?

Answer the new question directly, revise your plan if needed, and continue. An interruption may move the station forward; it is not necessarily a sign that your answer was wrong.

### Do adult neurology candidates need to rehearse pediatric cases?

Yes. The published applied-examination content includes pediatric disorders for both adult and pediatric candidates, so include them in your mock-case rotation.
