You can spend weeks reading neurology and still feel unprepared when five plausible answers appear on screen. The Specialty Certificate Examination (SCE) in Neurology tests whether you can apply specialist knowledge to a clinical problem—not simply recognise a diagnosis. As of October 2026, it consists of two computer-based papers, each with 100 best-of-five questions and three hours to complete them. Build your preparation around that task. [\[1\]](#cite-1 "Reference [1]")

Let the Blueprint Set Your Priorities
-------------------------------------

Download the Federation’s Neurology SCE blueprint before choosing a textbook chapter or question set. Its indicative 200-question distribution gives 26 questions to specific presentations, 24 to inflammatory and infectious disorders, and 20 each to cognition and consciousness, epilepsy, and movement disorders. The numbers may vary slightly, and topics are spread across both papers; use them to allocate study time, not to predict question order. [\[2\]](#cite-2 "Reference [2]")

### Turn Categories Into Decisions

Make a one-page grid with three columns: blueprint category, confidence, and next action. Give substantial categories repeated practice, then reserve short sessions for areas your daily work rarely exposes you to—such as neuro-otology, neurogenetics, pregnancy or functional disorders. Do not omit a smaller category because it appears less often.

- For **epilepsy**, practise choosing the next investigation or management step from a vignette, not just naming seizure types.
- For **specific presentations**, work through localisation before looking at the options: cranial nerve, root, cord or peripheral nerve?
- For **diagnostics**, interpret an image or neurophysiology finding alongside the clinical stem rather than studying it in isolation.

The regulations state that questions draw on UK practice, including NICE and SIGN guidance. If you practise outside the UK, check a relevant UK guideline when a management answer conflicts with your local pathway. [\[1\]](#cite-1 "Reference [1]")

Use an Eight-Week Study Schedule Template
-----------------------------------------

An eight-week plan is a template, not a minimum requirement. If your rota disrupts a week, retain the mixed-question and review sessions; shorten reading before you abandon feedback on errors.

WhenMain taskEvidence of progressWeeks 1–2Map the blueprint; attempt mixed questions and identify weak categoriesRecord errors by topic and reasonWeeks 3–5Rotate major categories; revisit weak answers after several daysExplain the best answer without notesWeeks 6–7Complete timed, mixed 100-question papers; rehearse a two-paper dayTrack unfinished questions and recurring errorsWeek 8Review the error log, UK guidance and official sample questionsResolve persistent answer-choice confusions

On a working day, try 20–30 best-of-five questions followed by a focused review. For each miss, write one line: the clue you overlooked, why your choice was less good, and the rule you will use next time. Reattempt selected errors several days later. Repeated testing with feedback and spaced repetition have improved knowledge retention in studies of medical learners and physicians; they are a better use of your review time than immediately rereading every explanation. [\[3\]](#cite-3 "Reference [3]")

> **Pro tip:** Classify an error as a knowledge gap, misread stem, incorrect localisation or outdated management assumption. Each needs a different fix; another random question set will not necessarily address it.

Train for the Best-of-Five Decision
-----------------------------------

The Federation describes one correct answer and four closely related, plausible alternatives. Practise identifying what the stem actually asks—diagnosis, investigation, treatment or prognosis—before weighing options. This prevents you from selecting a true statement that does not answer the question. [\[4\]](#cite-4 "Reference [4]")

1. Read the final sentence first to identify the required decision.
2. Extract the discriminating findings: tempo, localisation, associated features, test results and relevant context.
3. Predict an answer before inspecting the five options.
4. Compare the two strongest options against the exact wording; choose the **best** answer for this patient.

Use the Federation’s sample questions to calibrate this reasoning. A question bank supplies volume, but check its explanations against a reliable neurology text and current UK guidance when answers are disputed. Use a course only for a demonstrated weak area, and ask a study partner to defend the runner-up option rather than simply announce the correct letter. [\[4\]](#cite-4 "Reference [4]")

Rehearse the Two-Paper Day
--------------------------

Three hours for 100 questions averages **108 seconds per question**. In practice, move promptly through clear answers and preserve time for stems with imaging or competing management options. At question 50, check whether approximately half your time remains; adjust your pace instead of waiting until the final screen. [\[5\]](#cite-5 "Reference [5]")

Complete at least one rehearsal of both papers under timed conditions, with the scheduled one-hour break between them. Review whether your accuracy or pace deteriorates in Paper 2, then target that pattern in your next practice session. Because incorrect answers attract no negative mark, select an answer for every question before time expires; return to uncertain items only if time allows. [\[5\]](#cite-5 "Reference [5]")

> **Pro tip:** A familiar percentage from a question bank is not a guaranteed pass target. The SCE pass standard is equated, so the percentage required can differ between exam sittings. [\[6\]](#cite-6 "Reference [6]")

Common Pitfalls to Avoid
------------------------

- **Reading without decisions:** After each short textbook or guideline section, answer related clinical questions and explain why the distractors lose.
- **Revising only your subspecialty:** Compare question-bank performance with every blueprint category, including special groups and rehabilitation.
- **Memorising an old pathway:** Check UK guidance when a treatment question depends on current practice.
- **Practising one paper only:** Rehearse sustained decision-making across both three-hour papers.

Key Takeaways: Act This Week
----------------------------

- Download the official blueprint and mark your three weakest categories.
- Complete one mixed question set and start a four-category error log.
- Schedule two timed 100-question practices and one full two-paper rehearsal.
- Check the current exam regulations and your test-centre arrangements before exam day. UK and international SCE sittings moved to in-centre delivery in 2026. [\[7\]](#cite-7 "Reference [7]")

Make the Next Question Useful
-----------------------------

You do not need to master every neurology text before starting questions. Start with the blueprint, practise decisions under time pressure, and let each error tell you what to revise next.

    Frequently Asked Questions
----------------------------

 ### Should I divide revision time equally between Neurology SCE topics?

No. Use the official blueprint’s indicative question distribution, then adjust for your weak areas. Keep some practice for every category. [\[2\]](#cite-2 "Reference [2]")

### How should I review a best-of-five question I answered incorrectly?

Identify the missed clinical clue, explain why your choice was less appropriate, and reattempt the question after several days.

### Is there negative marking in the Neurology SCE?

No. Correct answers earn one mark; incorrect or unanswered questions earn none. Answer every question before time runs out. [\[1\]](#cite-1 "Reference [1]")

### Is passing the Neurology SCE enough to complete UK specialist training?

No. The SCE is an assessment requirement, not a substitute for the other requirements for completion of specialist training. [\[1\]](#cite-1 "Reference [1]")
