About the ABPS Exam
---------------------

The American Board of Physician Specialties (ABPS) offers board certification in Anesthesiology through its Board of Certification in Anesthesiology (BCA). The certification process is a rigorous two-part assessment consisting of a computer-based written examination and an oral certification examination. It is designed to validate a physician's comprehensive knowledge, clinical judgment, and mastery of anesthesiology.

### Eligibility

Candidates must hold an MD or DO degree, possess an active and unrestricted medical license in the U.S., its territories, or Canada, and have completed an ACGME, AOA, or RCPSC-accredited residency in anesthesiology. Verification of staff privileges and adherence to the ABPS Code of Ethics are also required.

### Exam Format

The ABPS Anesthesiology certification consists of two parts. Part I is a computer-based written exam consisting of multiple-choice questions (MCQs). Part II is an oral certification examination, which evaluates applied clinical judgment and case management, available only after passing the written exam.

### Duration

The written examination generally takes 4 to 5 hours to complete. The oral examination involves multiple timed clinical case scenarios and typically spans a half-day session.

### Passing Criteria

The written exam uses a scaled scoring system with a minimum passing standard established through psychometric equating. The oral examination is graded by a panel of board-certified examiners evaluating specific competency domains such as clinical reasoning, complication management, and patient safety.

### Exam Fees

Candidates typically pay a non-refundable application fee of approximately $850, an initial written exam fee around $1,200, and a subsequent oral exam fee of roughly $1,600. Candidates should verify current fee structures directly with the ABPS.

### Exam Frequency

The written exam is administered once annually during a defined testing window in May. The oral examination is offered once yearly, typically in the fall.

### Registration Process

Registration requires creating an account on the ABPS portal, completing a detailed application, and submitting primary source verifications (medical degree, residency certificates, license, and letters of recommendation). Applications must be approved by the BCA credentials committee before scheduling.

### Oral Certification Examination

The ABPS Anesthesiology Oral Certification Examination evaluates a candidate's clinical judgment, adaptability, and decision-making skills in real-time scenarios. Administered by the Board of Certification in Anesthesiology, it focuses on the practical application of medical knowledge rather than rote memorization. Candidates are presented with complex clinical cases and must articulate their management plans, anticipating potential complications and justifying their clinical choices.

How to Prepare for the ABPS
-----------------------------

A practical guide to help you plan your ABPS preparation effectively

### Recommended Timeline

6 to 9 months of structured study for the written examination, followed by 3 to 4 months of intensive, case-based practice for the oral examination.

      ### Study Tips

- Utilize highly rated Q-banks to practice multiple-choice questions in timed, exam-like conditions.
- Review current American Society of Anesthesiologists (ASA) practice guidelines and algorithms.
- For the oral exam, form study groups to conduct mock oral sessions, focusing on thinking out loud.
- Develop a structured, systematic approach to answering oral scenarios (e.g., Pre-op, Intra-op, Post-op).
- Focus heavily on the 'why' behind clinical decisions rather than just rote memorization.
- Familiarize yourself with the management of rare but life-threatening crises like malignant hyperthermia and LAST.

       ### High-Yield Topics

- Difficult airway management algorithms and awake intubation
- Local Anesthetic Systemic Toxicity (LAST) and lipid emulsion therapy
- Obstetric hemorrhage and preeclampsia management
- Perioperative management of pacemakers and ICDs
- Pediatric fluid resuscitation and congenital airway anomalies
- Malignant hyperthermia diagnosis and treatment
- Hemodynamic management in aortic stenosis vs. mitral regurgitation

      ### Common Pitfalls

- Poor time management during the computer-based written exam.
- Failing to verbalize the differential diagnosis and thought process during the oral examination.
- Defending a poor clinical decision in the oral exam rather than adapting to new information provided by the examiner.
- Under-preparing for basic science and pharmacology concepts.
- Overlooking the importance of thorough preoperative evaluation in mock oral cases.

    Frequently Asked Questions
----------------------------

Everything you need to know about ABPS preparation

What is the difference between ABPS and ABA certification?

The American Board of Physician Specialties (ABPS) and the American Board of Anesthesiology (ABA) are two distinct certifying bodies in the United States. Both require rigorous examinations and completion of an accredited residency, but they have different organizational structures, exam formats, and eligibility pathways. Some physicians choose ABPS as an alternative pathway to board certification.

Can I take the oral exam before passing the written exam?

No, candidates must successfully pass the ABPS Anesthesiology computer-based written examination before they are eligible to register for and take the oral certification examination.

How long do I have to pass the oral exam after completing the written exam?

Generally, candidates have a specified window (often up to 3-5 years or a set number of attempts) to pass the oral examination after successfully completing the written test. Candidates should verify the exact time limit in the current ABPS BCA handbook.

Is the ABPS Anesthesiology certification recognized nationwide?

Yes, ABPS is a recognized multi-specialty certifying body in the United States. Its certifications are widely accepted by hospitals, health systems, and credentialing committees, though candidates should always confirm specific credentialing requirements with their employers.

Can Canadian-trained anesthesiologists apply for this exam?

Yes, physicians who have completed anesthesiology residency training accredited by the Royal College of Physicians and Surgeons of Canada (RCPSC) are generally eligible to apply, provided they meet all other licensure and practice requirements.

What happens if I fail the written or oral exam?

Candidates who fail either portion of the exam may apply to retake it during a future testing window. A re-examination fee applies, and there may be limits on the total number of allowable attempts within a specific timeframe.

How should I structure my preparation for the oral boards?

Preparation for the oral exam should transition away from multiple-choice questions toward case-based learning. Practicing mock scenarios with colleagues or mentors, speaking your thought processes out loud, and using resources like Yao & Artusio's are highly recommended strategies.

Are there maintenance of certification requirements?

Yes, once certified, diplomates must participate in the ABPS continuous certification process, which typically involves demonstrating ongoing continuing medical education (CME), active practice, and periodically passing a recertification assessment.
