 About the RCPSC Exam
----------------------

The RCPSC Emergency Medicine Specialty Examination is the national certification exam required to obtain Fellowship (FRCPC) in Emergency Medicine in Canada. It rigorously evaluates the advanced clinical competence, knowledge, and problem-solving skills necessary for independent practice as an emergency medicine specialist. Successful completion designates the physician as a recognized Royal College specialist in Emergency Medicine.

### Eligibility

Candidates must have successfully completed a Royal College-accredited five-year residency program in Emergency Medicine. Internationally trained specialists may also be eligible if their training has been assessed and approved by the RCPSC via the credentialing process or Practice Eligibility Route.

### Exam Format

The examination consists of two main parts: a comprehensive written component (delivered on a computer-based platform at regional centers) and an applied/oral clinical examination (often conducted virtually), which focuses on real-time case management, patient interaction, and clinical reasoning.

### Duration

The written component typically spans two days, consisting of multiple papers. The applied/oral examination involves several structured clinical encounters conducted over a half-day session.

### Passing Criteria

Candidates must pass both the written and the applied components independently. Scoring utilizes standardized rubrics and a modified Angoff method to determine the minimum passing standard. You cannot proceed to the applied exam without passing the written exam.

### Exam Fees

Examination fees generally range from CAD 3,500 to CAD 4,500 for the combined written and applied components. Candidates should verify the exact and current fee schedule on the official RCPSC website.

### Exam Frequency

The RCPSC Emergency Medicine exam is held once annually.

### Registration Process

Registration is completed online via the Royal College portal. Candidates must first have their preliminary assessment of training approved before they are granted access to register and pay for the examination.

How to Prepare for the RCPSC
------------------------------

A practical guide to help you plan your RCPSC preparation effectively

### Recommended Timeline

12 to 18 months of highly structured study, typically beginning in the PGY-4 year and intensifying throughout PGY-5.

      ### Study Tips

 - Form a dedicated study group of 2-4 peers early in your PGY-4 year to maintain accountability and practice oral exams.
- Simulate the time constraints of the written exam by practicing short-answer questions (SAQs) and focusing on concise, point-form answers.
- Conduct regular mock oral examinations with staff physicians to refine your clinical reasoning and communication under pressure.
- Review Canadian-specific guidelines closely, particularly from the Canadian Association of Emergency Physicians (CAEP) and Thrombosis Canada.
- Develop systematic, out-loud approaches for interpreting ECGs, diagnostic imaging, and lab results for the applied component.
- Familiarize yourself with the virtual exam environment by practicing oral cases over video conferencing platforms.

       ### High-Yield Topics

 - Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) protocols
- Advanced Trauma Life Support (ATLS) and trauma team leadership
- Acute Coronary Syndromes and advanced ECG interpretation
- Point-of-Care Ultrasound (POCUS) integration into acute care
- Clinical toxicology and specific antidote management
- Airway management algorithms, including the difficult and failed airway

      ### Common Pitfalls

 - Failing to answer the specific question asked in written SAQs, leading to wasted time on irrelevant information.
- Poor time management during the written examination, leaving the final questions blank.
- In the applied exam, failing to utilize closed-loop communication or failing to verbalize physical examination steps clearly.
- Neglecting 'minor' syllabus areas such as disaster medicine, administration, or pre-hospital care, which frequently appear on the exam.
- Over-studying rare zebras at the expense of mastering the immediate life-saving management of common critical presentations.

     Frequently Asked Questions
----------------------------

Everything you need to know about RCPSC preparation

 ### What is the difference between FRCPC and CCFP(EM)?

FRCPC in Emergency Medicine is a five-year specialty training program focused on tertiary care, complex trauma, research, and academic leadership. CCFP(EM) is a one-year certificate of added competence following a two-year Family Medicine residency, typically geared toward community or regional emergency practice.

### Can I take the applied (oral) exam if I fail the written exam?

No. Candidates must successfully pass the written component to be eligible to proceed to the applied/oral examination. If you fail the written component, you must retake it in a subsequent year.

### How many attempts are allowed for the RCPSC Emergency Medicine exam?

Candidates generally have a multi-year window of eligibility during which they can attempt the examination. The exact number of attempts may be subject to specific Royal College policies, and candidates needing multiple retakes should consult the RCPSC for any required remediation pathways.

### Are international medical graduates (IMGs) eligible to take the exam?

Yes, internationally trained physicians can challenge the exam if their training is deemed equivalent. This typically requires applying through the Approved Jurisdiction Route or the Practice Eligibility Route and passing a credentialing assessment by the Royal College.

### How much does the RCPSC EM examination cost?

The total cost for the examination generally ranges between CAD 3,500 and CAD 4,500. This fee covers both the written and applied components, but candidates should always confirm the current fee schedule on the official RCPSC website.

### What format are the written questions?

The written component typically utilizes a mix of multiple-choice questions (MCQs) and short-answer questions (SAQs). The SAQs require concise, specific answers and often feature clinical scenarios, laboratory data, or imaging.

### Do I need to travel to Ottawa for the examination?

Currently, the written component is administered at various regional computer-based testing centers across Canada. The applied/oral component has largely been transitioned to a virtual format, meaning candidates may not need to travel to Ottawa, though they should verify the format for their specific exam year.

### When do I need to register for the exam?

Registration usually opens in the fall of the year preceding the examination. Preliminary assessment of training must be completed and approved prior to registration, so candidates should begin the paperwork months in advance.
