 American Board of Family Medicine

      American Board of Family Medicine

       USA

ABFM sets the certification standards and exam requirement for U.S. family physicians seeking to earn and maintain board certification. Diplomates can meet the exam requirement through the Family Medicine Certification Longitudinal Assessment (FMCLA) or by taking the Family Medicine Certification Examination (one-day exam); residents pursuing initial certification must pass the one-day exam. Beginning in 2025, ABFM transitioned diplomates into a Continuous Certification 5‑Year Cycle built around ongoing participation in certification activities and cognitive assessment.

 About the ABFM Exam
---------------------

The American Board of Family Medicine (ABFM) certification exam sets the national standard for family physicians in the United States. It evaluates a physician's clinical knowledge, diagnostic skills, and ability to provide comprehensive, continuous care across all life stages. Diplomates can achieve or maintain certification via the traditional One-Day Family Medicine Certification Examination or, for continuous certification, the Family Medicine Certification Longitudinal Assessment (FMCLA).

### Eligibility

For initial certification, candidates must have successfully completed an ACGME-accredited Family Medicine residency program and hold a valid, unrestricted medical license in the US or Canada. Diplomates participating in the Continuous Certification 5-Year Cycle must meet ongoing CME, professionalism, and knowledge assessment requirements to remain eligible.

### Exam Format

The assessment is offered in two formats: a One-Day Exam consisting of secure, computer-based multiple-choice questions (MCQs) taken at a Prometric test center without references, and the FMCLA, a quarterly online assessment delivering 25 timed MCQs per quarter with references allowed.

### Duration

The One-Day Examination takes approximately 8.5 hours, including scheduled breaks. The FMCLA provides candidates 5 minutes to answer each of the 25 questions distributed quarterly.

### Passing Criteria

ABFM uses a criterion-referenced grading system with a scaled minimum passing score. For the One-Day Exam, the passing standard is determined psychometrically; for the FMCLA, successful completion over a 3- to 4-year period to the passing standard satisfies the requirement.

### Exam Fees

Initial certification fees are approximately $1,300 USD. Diplomates maintaining certification typically pay an annual Continuous Certification fee of around $200-$250 USD, which covers the cost of ongoing assessments like the FMCLA.

### Exam Frequency

The One-Day Examination is offered twice a year, typically in April and November. The FMCLA delivers questions continuously on a quarterly schedule.

### Registration Process

Candidates must register online through the ABFM Physician Portfolio portal. Registration requires updating medical license information, confirming residency completion (for initial certification), and submitting the required fees.

 ABFM Exam Structure
---------------------

Our materials cover all parts of the ABFM examination

### Family Medicine Certification Examination (One-Day Exam)

The ABFM One-Day Certification Examination is a comprehensive assessment designed to evaluate a physician's diagnostic abilities, clinical judgment, and management of scenarios commonly encountered in family medicine. It ensures candidates possess the requisite knowledge and awareness of current best practices necessary for board certification and the delivery of high-quality, comprehensive patient care.

Computer-based multiple-choice questions (single best answer) Approximately 7 to 9 hours (includes four 95-minute testing sections and 100 minutes of pooled break time) Pass: Reported as a scaled score ranging from 200 to 800, with a minimum passing standard (MPS) determined by psychometric standard-setting (historically around 380).

  Key Topics

Cardiovascular Respiratory Musculoskeletal Endocrine Gastrointestinal Psychogenic / Psychiatry +1 more

### Family Medicine Certification Longitudinal Assessment

The Family Medicine Certification Longitudinal Assessment (FMCLA) is a flexible, continuous certification pathway designed as an alternative to the traditional one-day ABFM examination. It evaluates a physician's clinical knowledge and decision-making skills over a three- to four-year period using a quarterly question-delivery model. By allowing the use of everyday clinical references and providing immediate feedback, the FMCLA closely mirrors real-world practice while promoting ongoing learning and knowledge retention.

Online, open-book longitudinal assessment consisting of 25 multiple-choice questions per quarter. 5 minutes per question (25 questions per quarter), administered continuously over a 3- to 4-year cycle. Pass: Criterion-referenced passing standard determined by the ABFM, evaluated cumulatively after the completion of the required 300 questions.

How to Prepare for the ABFM
-----------------------------

A practical guide to help you plan your ABFM preparation effectively

### Recommended Timeline

3-6 months of dedicated, structured study for the One-Day Exam; ongoing weekly review for the FMCLA.

      ### Study Tips

 - Master the USPSTF A and B recommendations, as these are heavily tested across multiple domains.
- Use comprehensive question banks that simulate the ABFM exam interface and timing.
- Review the latest American Academy of Family Physicians (AAFP) clinical practice guidelines.
- Practice time management for the One-Day Exam, aiming for roughly one minute per question.
- For the FMCLA, bookmark reliable, quick-reference clinical tools (like UpToDate or AAFP articles) to use during the 5-minute per question window.
- Identify your weakest organ systems early using a diagnostic practice test and tailor your study plan accordingly.

       ### High-Yield Topics

 - USPSTF screening guidelines (Cancer, Cardiovascular, Metabolic)
- Diabetes pharmacotherapy and treatment algorithms
- Hypertension guidelines and medication side effects
- Pediatric immunization schedules and catch-up rules
- Asthma and COPD step-up therapy protocols
- Common dermatologic presentations and their management

      ### Common Pitfalls

 - Failing to review maternity and pediatric care, which often make up a larger portion of the exam than adult-only practitioners expect.
- Relying on outdated clinical guidelines instead of the most current evidence-based medicine.
- Overthinking straightforward questions and second-guessing initial correct answers.
- Poor time management on the One-Day Exam, leading to rushed or unanswered questions in the final blocks.
- For FMCLA, spending too much time searching references and running out of the 5-minute time limit.

    Frequently Asked Questions
----------------------------

Everything you need to know about ABFM preparation

What is the difference between the One-Day Exam and the FMCLA?

The One-Day Exam is an 8.5-hour, secure, closed-book test taken at a Prometric center and is mandatory for initial certification. The FMCLA is an open-book, longitudinal assessment delivering 25 questions per quarter over 3-4 years, available only to current diplomates for maintaining certification.

Who is eligible to take the ABFM initial certification exam?

Physicians who have successfully completed an ACGME-accredited Family Medicine residency and possess a valid, unrestricted license to practice medicine in the United States or Canada are eligible.

What is the pass rate for the ABFM certification exam?

Historically, the ABFM One-Day Exam boasts a high pass rate for first-time takers completing a US residency, typically ranging between 85% and 90%. However, pass rates fluctuate slightly each year based on candidate performance and standard-setting methodologies.

Are references allowed during the ABFM exam?

For the One-Day Certification Examination at Prometric centers, no outside resources or references are permitted. However, candidates participating in the online FMCLA may use reference materials, though they must adhere to the 5-minute time limit per question.

What happens if I fail the ABFM exam?

Candidates who fail the initial certification exam can retake it during a subsequent testing window (April or November). There is no lifetime limit on the number of attempts, but candidates must meet all continuous eligibility requirements and pay the registration fee for each attempt.

How long is my ABFM certification valid?

Beginning in 2025, ABFM transitioned to a Continuous Certification 5-Year Cycle. Instead of a hard 10-year expiration date, diplomates maintain their certification continuously by fulfilling ongoing assessment (like FMCLA) and professional development requirements every five years.

Can I switch from the FMCLA to the One-Day Exam?

Yes. Diplomates who are enrolled in the FMCLA can choose to take the One-Day Exam instead if they prefer to meet their cognitive assessment requirement in a single sitting, though they should verify the specific transition process with the ABFM.

How are the questions distributed across different medical topics?

The exam blueprint is heavily weighted toward general primary care, with major sections covering cardiovascular (10-14%), respiratory (7-11%), and musculoskeletal (7-11%) systems. It also includes significant proportions of pediatric, geriatric, and maternity care questions to reflect the full scope of family medicine.
