 About the MRCGP Exam
----------------------

The Membership of the Royal College of General Practitioners (MRCGP) is the integrated assessment system required for General Practice (GP) specialty training in the UK. It ensures that a doctor has met the standard for independent general practice. The exam consists of three mandatory components: the Applied Knowledge Test (AKT), the Simulated Consultation Assessment (SCA), and continuous Workplace-Based Assessment (WPBA).

### Eligibility

Candidates must be registered GP trainees enrolled in a UK General Practice specialty training programme. The AKT is typically taken in the ST2 or ST3 year, while the SCA is usually undertaken during the ST3 year. Candidates must have an active RCGP trainee portfolio to register.

### Exam Format

The MRCGP has three parts. The AKT is a computer-based written exam consisting of 160 Single Best Answer (SBA) and Extended Matching Questions (EMQ). The SCA is a clinical exam involving 12 simulated remote patient consultations. The WPBA is a programmatic portfolio assessment evaluated continuously over the 3-year training period.

### Duration

The AKT takes 3 hours and 10 minutes. The SCA consists of 12 cases, each lasting 12 minutes, plus 3 minutes of reading time per case, taking approximately 3 hours overall. WPBA is continuous throughout the three years of GP training.

### Passing Criteria

The AKT pass mark varies per diet using the Angoff method to ensure fairness. The SCA is marked using the borderline regression method across three domains: Data Gathering, Clinical Management, and Interpersonal Skills. WPBA requires a final Annual Review of Competence Progression (ARCP) Outcome 6.

### Exam Fees

Exam fees are approximately £470 for the AKT and £1,180 for the SCA, in addition to annual trainee portfolio fees. Candidates should verify current fees on the official RCGP website as they are subject to annual review.

### Exam Frequency

The AKT is held three times a year (typically January, April, and October). The SCA is held much more frequently, typically with 9 diets per year, excluding the late summer.

### Registration Process

Candidates must register online via their RCGP account. Booking windows for each exam diet open for a specific period, usually 6-8 weeks before the exam. Candidates must meet eligibility criteria for their training year before booking.

How to Prepare for the MRCGP
------------------------------

A practical guide to help you plan your MRCGP preparation effectively

### Recommended Timeline

4-6 months of focused study for the AKT, and 3-6 months of continuous consultation practice for the SCA.

      ### Study Tips

 - For the AKT, base your clinical knowledge firmly on current NICE Clinical Knowledge Summaries (CKS) rather than secondary care textbooks.
- Do not neglect the non-clinical sections (Admin and Statistics) of the AKT; these make up 20% of the exam and are highly predictable.
- For the SCA, practice 12-minute simulated consultations with colleagues, strictly timing yourself to ensure you cover data gathering, clinical management, and interpersonal skills.
- Record your real-life consultations (with patient consent) and review them with your clinical supervisor to identify communication weaknesses.
- Keep your WPBA FourteenFish portfolio updated weekly; do not leave clinical encounter logs and reflections to the end of the rotation.
- Memorise key algorithms, such as the UKMEC categories for contraception and DVLA fitness to drive rules.

       ### High-Yield Topics

 - NICE Clinical Knowledge Summaries (CKS)
- DVLA Medical Standards of Fitness to Drive
- UKMEC (UK Medical Eligibility Criteria for Contraceptive Use)
- Medical Statistics and Critical Appraisal
- BNF prescribing guidelines and interactions
- Child and Adult Safeguarding procedures

      ### Common Pitfalls

 - Failing to manage time during the AKT, leading to unattempted questions at the end of the 160-item paper.
- In the SCA, rushing the clinical management plan without negotiating it with the patient or checking their understanding.
- Over-investigating or referring to secondary care too quickly in the SCA, rather than demonstrating safe, independent GP management.
- Underestimating the difficulty of the Evidence-Based Practice and GP Administration sections in the AKT.
- Leaving WPBA log entries, mini-CEXs, and CbDs until the weeks just before the ARCP panel.

      ### Recommended Resources

 - NICE Clinical Knowledge Summaries (CKS)
- RCGP Curriculum and official RCGP exam resources
- FourteenFish (for WPBA portfolio and official RCGP SCA preparation materials)
- Passmedicine, Emedica, or BMJ OnExamination (for AKT question banks)
- BNF and BNF for Children

    Frequently Asked Questions
----------------------------

Everything you need to know about MRCGP preparation

What is the difference between the AKT and the SCA in the MRCGP?

The AKT (Applied Knowledge Test) is a written, computer-based exam that tests the knowledge base required for general practice. The SCA (Simulated Consultation Assessment) is a clinical exam that assesses your ability to integrate that knowledge into patient consultations, focusing on communication, data gathering, and clinical management.

When is the best time to sit the MRCGP AKT?

Most GP trainees choose to sit the AKT during their ST2 year. This allows enough time to retake the exam if necessary, and gets the heavy knowledge-based preparation out of the way before focusing on the SCA in ST3.

How many attempts are allowed for the MRCGP exams?

Candidates are generally allowed a maximum of four attempts for both the AKT and the SCA. However, taking a fourth attempt is often subject to educational supervisor approval and potential extension of training time by the local Deanery.

What is the format of the new MRCGP SCA?

The Simulated Consultation Assessment (SCA) consists of 12 remote simulated consultations, each lasting 12 minutes. Candidates consult with professional role-players via an online platform, usually undertaken at a local GP surgery or approved test center.

How long does it take to prepare for the MRCGP AKT?

Most successful candidates recommend 4 to 6 months of structured study. Preparation usually involves completing thousands of practice questions from standard question banks alongside reading NICE CKS guidelines.

What is the pass rate for the MRCGP exams?

Historically, the pass rate for the AKT hovers around 65% to 75% per diet, while the clinical assessment pass rates are generally similar but can vary. Pass marks fluctuate each diet based on standard-setting methodologies.

Is the WPBA a formal exam?

No, the Workplace-Based Assessment (WPBA) is not a single sit-down exam. It is a continuous, programmatic assessment over the three years of GP training, requiring trainees to log clinical cases, reflections, and supervisor assessments into the FourteenFish e-portfolio.

How much does the MRCGP cost?

The combined exam fees for the AKT and SCA typically exceed £1,600. When factoring in the annual trainee portfolio fees and potential preparation courses, the total cost is significant. Candidates should verify precise, current fees on the RCGP website.

Do I need to pass the AKT before sitting the SCA?

While it is not strictly mandatory in all Deaneries to have passed the AKT before attempting the SCA, it is highly recommended. The clinical knowledge required for the AKT forms the foundation for safe clinical management in the SCA.

How is the MRCGP AKT scored?

The AKT is scored electronically with no negative marking. The pass mark is determined for each specific diet using the Angoff method, ensuring that variations in paper difficulty do not unfairly penalize candidates.
