About the FCPS Exam
---------------------

The Fellowship of the College of Physicians and Surgeons Pakistan (FCPS) in Obstetrics & Gynecology is a premier postgraduate medical qualification. It certifies doctors as specialist consultants through a rigorous three-stage assessment: Part I, the Intermediate Module (IMM), and Part II. The pathway ensures comprehensive knowledge and clinical competence in women's health.

### Eligibility

Candidates must hold a PMDC-recognized medical degree (MBBS) and have completed a one-year house job to sit for Part I. For IMM and Part II, candidates must complete 2 to 4 years of structured, CPSP-accredited training respectively, and submit approved supervisor logs.

### Exam Format

The exam is staged: Part I consists of multiple-choice questions (BCQs) on basic sciences. IMM features theory and TOACS. Part II includes theory papers (MCQs) followed by a comprehensive clinical exam involving long cases, short cases, and TOACS scored independently by paired examiners.

### Duration

Theory papers typically last 3 hours each. Clinical exams (TOACS, long, and short cases) for IMM and Part II are conducted over 1 to 2 days per candidate.

### Passing Criteria

Theory exams generally require a passing score of around 70-75%, based on CPSP's standardized grading system. For clinicals, candidates must achieve satisfactory scores across multiple independent examiner pairs.

### Exam Fees

Examination fees range from approximately PKR 15,000 for Part I to PKR 35,000 or more for Part II, excluding registration and late fees. Candidates should verify current fee structures directly with CPSP.

### Exam Frequency

Part I is typically held three to four times a year. IMM and Part II are generally conducted twice a year.

### Registration Process

Candidates must register online via the CPSP e-portal. The process requires uploading PMDC registration, training certificates, supervisor endorsements, completed e-logbooks for later stages, and the payment voucher.

FCPS Exam Structure
---------------------

Our materials cover all parts of the FCPS examination

### FCPS Part I

The FCPS Part 1 in Obstetrics & Gynecology is a foundational written examination conducted by the College of Physicians and Surgeons Pakistan (CPSP). It assesses a candidate's grasp of core basic medical sciences—such as anatomy, physiology, and pathology—specifically as they relate to women's reproductive health, childbirth, and fetal development. Passing this rigorous assessment is a mandatory prerequisite for medical graduates to enter formal clinical residency training in the specialty.

Computer-based examination comprising two theory papers (Paper I for general basic sciences and Paper II for specialty-specific basic sciences). Each paper contains 100 One-Best-Answer Multiple Choice Questions (MCQs) with no negative marking. 2 to 2.5 hours per paper (Total examination duration is 4 to 5 hours, usually completed in a single day). Pass: Candidates must achieve a minimum score of 75% in each of the two papers individually to pass.

  Key Topics

Anatomy and Embryology Physiology and Endocrinology Pathology Pharmacology Microbiology and Immunology Genetics and Biochemistry

### Intermediate Module (IMM)

The Intermediate Module (IMM) in Obstetrics and Gynecology is a critical mid-training assessment conducted by CPSP after the first two years of residency. It evaluates core clinical knowledge, evidence-based management, and fundamental procedural skills, serving as a mandatory prerequisite for advancing to the final phase of FCPS-II specialized training.

Computer-Based Written Examination consisting of Multiple Choice Questions (MCQs), followed by a Clinical Examination utilizing the Task-Oriented Assessment of Clinical Skills (TOACS) format. The written exam consists of two papers, each lasting 2 hours (120 minutes) for computer-based testing, with a short break in between. The subsequent TOACS component varies in duration based on the number of interactive and static stations. Pass: Candidates must achieve a combined score of 75% across both written MCQ papers to clear the theory component and become eligible for the TOACS clinical assessment.

  Key Topics

Intrapartum Care & Obstetric Emergencies Maternal Medicine & Antenatal Care General Gynecology Early Pregnancy Complications Reproductive Endocrinology & Infertility Gynecological Oncology +2 more

### FCPS Part II

The FCPS Part II in Obstetrics and Gynecology is the final certification examination by the College of Physicians and Surgeons Pakistan. It assesses advanced theoretical knowledge, clinical competence, surgical judgment, and comprehensive patient management required to practice independently as a consultant obstetrician and gynecologist.

Written papers (MCQs and Short Essay Questions) and a multi-station Clinical/Oral examination (TOACS, long cases, short cases, and viva voce). Multiple days (Two 3-hour written papers followed by clinical examinations on separately scheduled days). Pass: Candidates must pass the written exam to qualify for clinicals; clinical passing requires independent satisfactory scoring by paired examiners, typically equating to a 60% standard.

  Key Topics

Maternal-Fetal Medicine General Obstetrics General Gynecology Gynecological Oncology Reproductive Endocrinology and Infertility Urogynecology

How to Prepare for the FCPS
-----------------------------

A practical guide to help you plan your FCPS preparation effectively

### Recommended Timeline

6-12 months of intensive, structured study per examination stage (Part I, IMM, Part II).

      ### Study Tips

- Form study groups for TOACS and OSCE practice to improve verbal presentation skills.
- Extensively practice past BCQs (Best Choice Questions) to understand CPSP's testing pattern.
- Integrate local Society of Obstetricians and Gynaecologists of Pakistan (SOGP) guidelines alongside RCOG/ACOG recommendations.
- Schedule mock clinical exams with senior registrars or supervisors to simulate the pressure of paired examiners.
- Maintain and review your e-logbook regularly, as real clinical cases provide the best foundation for the Part II long case.
- Focus heavily on the rationale behind management steps, as examiners test decision-making and patient safety.

       ### High-Yield Topics

- Postpartum Hemorrhage (PPH) prevention and surgical management
- Eclampsia and severe preeclampsia protocols
- Interpretation of CTG (Cardiotocography) and partograms
- Pelvic floor anatomy and ureter relations
- Infertility workup and ovulation induction algorithms
- Cervical cancer screening and management of abnormal smears

      ### Common Pitfalls

- Neglecting basic sciences in Part II preparation, forgetting that anatomy and physiology are heavily tested in surgical rationale.
- Poor time management during the TOACS stations, leading to missed critical counseling steps.
- Presenting clinical cases like a textbook rather than tailoring management to the specific patient scenario.
- Focusing only on theory and starting clinical practice too late in the preparation timeline.
- Failing to demonstrate safe practice and awareness of ethical guidelines during viva voce.

      ### Recommended Resources

- Dewhurst's Textbook of Obstetrics & Gynaecology
- Williams Obstetrics
- Berek & Novak's Gynecology
- RCOG Green-top Guidelines and NICE Guidelines
- Ten Teachers (Obstetrics and Gynaecology) for foundational review
- Past CPSP BCQ compilation books for Part 1 and IMM

     Frequently Asked Questions
----------------------------

Everything you need to know about FCPS preparation

### What is the difference between FCPS Part 1, IMM, and Part 2?

FCPS Part I is a foundational exam testing basic medical sciences, taken early in training. The Intermediate Module (IMM) is an assessment taken midway through residency to ensure clinical progress. Part II is the final exit exam, combining advanced theory and complex clinical assessments to grant the fellowship.

### How many attempts are allowed for FCPS Part 1?

Historically, CPSP allows multiple attempts for Part I, but candidates must pass it before they can begin their formal structured training for the fellowship. It is highly recommended to check the most current attempt limits and validity of the MBBS house job with CPSP.

### Is the FCPS OBGYN recognized internationally?

Yes, the FCPS qualification is highly respected and recognized in many Middle Eastern countries (like Saudi Arabia and the UAE) and is considered equivalent to local specialist degrees in various international jurisdictions, facilitating global career mobility.

### What is the format of the clinical examination for Part II?

The clinical exam consists of a long case, several short cases, and a TOACS (Task-Oriented Assessment of Clinical Skills). These are evaluated by pairs of independent examiners to ensure objective scoring, focusing on history taking, examination, and management plans.

### What happens if I fail the clinical part but pass the theory?

Candidates who pass the theory component but fail the clinical examination generally retain their theory pass status for a limited number of consecutive clinical attempts. If they exhaust these attempts, they must retake the theory exam.

### How much does it cost to take the FCPS exams?

Exam fees vary by stage, generally ranging from PKR 15,000 for Part I up to PKR 35,000 or more for Part II. These figures exclude registration, late fees, and training enrollment costs, so candidates must check the latest CPSP fee structure.

### Are RCOG guidelines applicable for the FCPS exam?

Yes, examiners heavily rely on evidence-based medicine, and RCOG Green-top Guidelines are widely considered the gold standard for management protocols in the FCPS OBGYN exam, alongside local SOGP guidelines.

### How long does the entire FCPS training take?

The full pathway typically takes 4 to 5 years after the house job. This includes passing Part I, completing two years of training to clear IMM, and completing another two years of training before passing Part II.
